Inject test ServerConfig across orchestration and git layers

- add `ServerConfig.layerTest(cwd, stateDir)` to standardize test wiring
- require `ServerConfig` in `CodexTextGeneration` attachment path resolution
- update orchestration/projection tests and harness to use managed runtimes with explicit config
