## Description

---

In the "Install the plugin" section of `/mcp`, the two hosts' command cards stacked without a lead-in, and each card's caption renders at its bottom — so a reader scanning the section saw "Claude Code plugin" sitting directly above the Codex commands and read it as one Claude Code block containing `codex` lines.

Each command card now gets a one-line lead naming its host ("For Claude Code, run:" / "For Codex, run:") directly above the commands, so the pairing can't be misread regardless of where the card caption lands.

Content is unchanged — same commands, same cards, same labels. Verified with `npm test`: 79/79 green, including the test asserting each snippet connects to the plugin endpoint.
