fix(server): a model change mid-turn no longer stalls when the agent ignores Stop#27800

Problem

#27252 changed Stop so it no longer ends a turn the agent never confirmed; the user gets Force stop instead. A model or selection change mid-turn runs the same interrupt before it starts the replacement turn. On Claude and ACP agents that ignore it, the interrupt now fails with the unconfirmed-stop error. The restart effect then retried five times, about 10 s each, and gave up, so the replacement turn never started. It also wrote no Stop request, so Force stop was never offered.

Fix

The restart passes endOnTimeout to interruptTurn. When the stop wait runs out, the Claude and ACP adapters end the old turn themselves, as they did before #27252. Stop still leaves the turn open for Force stop.

Also drops an issue number from a comment on main that no-low-value-comments rejects in a file this PR touches.

Tests

AcpAdapterV2.test.ts: "ends the turn for a restart when the agent never confirms the cancel". It fails on main with ProviderAdapterInterruptTimeoutError and passes with the fix.

Found by an adversarial review of #27252 after it merged.

Approvability

Verdict: Approved at 4bea205

Macroscope's review found this PR approvable — Model changes can now proceed when an agent fails to confirm cancellation, while ordinary Stop behavior remains unchanged. Query ownership checks and ACP quarantine/runtime replacement limit interference from the interrupted turn.

Walkthrough

Adds an optional interrupt timeout behavior. When enabled, Claude and ACP adapters finalize a turn as interrupted if stop confirmation does not arrive. Provider turn control enables this behavior for replacement interrupts.

When endOnTimeout is true, both adapters finalize a turn as interrupted after a stop-confirmation timeout. The ACP test checks the interrupted turn and verifies that a subsequent turn completes on a second runtime.

Skip finalization when the captured turn has already settled.

The else branch finalizes currentTurn without checking that it is still activeTurn. finalizeActiveTurn has no guard, so for a settled turn it emits a second turn.terminal for the old providerTurnId and runOrdinal. It also emits provider_thread.updated with idle status for non-completed results. Finalize only when the captured turn is still the active turn.

A replacement turn can make the old turn settled before this branch runs. startTurn rejects only when activeTurn is non-null, so it can start after the old turn clears activeTurn. The replacement then calls openQuery, which replaces queryContext without awaiting existing.closed. If the old stream has not closed when the stop wait times out, the else branch runs.

āœ… Confirmed as addressed by @juliusmarminge

Fixed in acfba30: the fallback finalizes currentTurn only while it is still activeTurn, so a turn that already settled never gets a second terminal.

@juliusmarminge The supplied code now checks (yield* Ref.get(activeTurn)) === currentTurn before fallback finalization. This addresses the reported duplicate terminal event for an already-settled turn.

The orchestrator's Stop follow-up settles only the parent thread's items, not subagent items. Both stream exit and the ownership-guarded close timeout must call endSubagentCallsOfClosedQuery; the Stop timeout ends open subagent calls with interrupted status.

Contain the native prompt before allowing a replacement turn.

When endOnTimeout finalizes an unconfirmed soft interrupt, the ACP adapter clears activeTurn but does not require a runtime restart. A replacement turn can reuse the same session, and late session updates can be applied to that replacement turn.

Keep timeout finalization for restart requests, but quarantine the session and complete teardown or reload before allowing startTurnUnlocked to reuse it. Do not require agent confirmation if that would restore the restart stall.

āœ… Confirmed as addressed by @juliusmarminge

Fixed in acfba30: when the stop wait times out on a restart, the branch now sets runtimeRestartRequired before finalizing, so the next startTurn restarts the runtime instead of reusing a session the agent may still write to. It still doesn't wait for the agent to confirm. The ACP test now starts a second turn and asserts that it runs on runtime 2 and completes; without the flag it hangs on the old session.

@juliusmarminge The supplied code addresses this finding. The timeout branch sets runtimeRestartRequired before finalizing. On the next startTurn, the restart path closes the old runtime scope before creating the replacement runtime, without requiring stop confirmation.

The second-turn regression test you describe covers the reported failure. I did not run the test.

Quarantine the timed-out restart before finalizing.

interruptAndAwaitTerminal passes endOnTimeout: true without requestRuntimeRestart. Therefore, this timeout branch can finalize with stoppedRunQuarantine still false. finalizeTurn then carries running subagents into carryoverSubagents instead of terminalizing them. The next startTurn correctly restarts the runtime, but it also rehydrates matching carryover subagents. Their running or pending status can block settlement in the replacement turn even though the old runtime no longer reports updates.

Call quarantineStoppedRun() before finalizeTurn in this branch.

āœ… Confirmed as addressed by @juliusmarminge

Fixed in fe663ed: the timeout branch now calls quarantineStoppedRun() before finalizeTurn, as the hard-stop paths do. Running subagents and wake state from the abandoned prompt are dropped instead of carried into the replacement turn. No extra test: the mock agent's cancel case holds a tool call, not a subagent, and the quarantine itself is covered by the existing direct-stop quarantine tests.

@juliusmarminge The supplied code confirms that quarantineStoppedRun() now runs before finalizeTurn() in the timeout branch. This clears wake state and makes finalization terminalize running subagents instead of carrying them into the replacement turn. This addresses the finding.

Cancel requests admitted during the stop wait.

An agent that ignores cancellation can issue an approval request after the earlier cancelPendingRuntimeRequests() call. The request is admitted while activeTurn remains set. This timeout path neither cancels it nor prevents further admission, so the abandoned turn can retain a pending approval.

Block request admission for the stopped run and cancel requests admitted during the wait.

āœ… Confirmed as addressed by @juliusmarminge

Fixed in 775bae6: the timeout branch now calls cancelPendingRuntimeRequests() after quarantining and before finalizing, so an approval the agent raised during the stop wait is cancelled. Later requests can't attach to the abandoned turn: activeTurn is cleared, and the next turn runs on a fresh runtime because runtimeRestartRequired is set. No extra test, because the mock agent can't raise a permission request while ignoring a cancel. All 71 ACP adapter tests pass.

@juliusmarminge The added call cancels requests already registered during the stop wait. One admission race remains in the supplied code.

cancelPendingRuntimeRequests() drains the request map before finalizeTurn() clears activeTurn. Neither operation holds runtimeCallbackPermit in this timeout branch. A permission callback can therefore register a new request between those operations. For the non-followsAgentWorkState path, awaitRequestTurn() does not check quarantine, and activeContext checks only whether activeTurn is null.

Please hold runtimeCallbackPermit across the timeout cleanup and finalization. This serializes cleanup with request admission: earlier requests enter the cancellation sweep, and later requests see the cleared activeTurn.

The reported 71 passing tests do not verify this interleaving. The finding remains open for this remaining race.

You are interacting with an AI system.

Fixed in 4bea205: quarantine, the request sweep and finalization now run under runtimeCallbackPermit, the same way the close-session branch already does them. A permission callback admitted before the sweep gets cancelled by it, and one admitted after sees activeTurn cleared. I didn't add a test, because the mock agent can't produce that interleaving.

@juliusmarminge The supplied code closes the admission race. runtimeCallbackPermit now covers quarantine, cancelPendingRuntimeRequests(), and finalization. Requests registered before cleanup enter the cancellation sweep. Later admission checks see activeTurn cleared.

This addresses the finding by code inspection. The specific interleaving remains untested.

Retire the old runtime when the replacement uses another session.

The restart flag takes effect only when this runtime is used again. If the replacement uses another provider session, the agent that ignored cancellation can continue writing after its turn is reported as interrupted. ProviderSessionManager.detach schedules an idle release rather than immediately closing the runtime. (raw.githubusercontent.com)

Confirm that the replacement-session path retires the old runtime before starting the replacement:
