Fleet traces from the 24h after the 2026-10-29 perf batch (nightly 3904/3917) showed four remaining server hotspots. The biggest: nucbox-1's server heap spiked past 1 GB about 100 times a day, peaking at 3.1–4 GB. server.heap.spike suspects pointed at ProjectionStore.readCanonicalProjection inside command dispatch.

Settled subagents in command reads. One dispatcher thread there has 4,711 finished subagents holding 154 MB of JSON, 145 MB of it prompts. Every command on that thread (starting a queued run, a scheduled send, delegated completion) decoded all of them. readCommandProjection now passes omitSettledSubagents. That filter skips subagents that ended with an acknowledged, delivered or disposed completion, unless their run is active. It follows the pattern #19675 used for finished tool nodes.

  • Command paths read subagents only to look up pending or claimed delivery tasks, scan active work, or cascade within the active run. Acknowledging removes a task from its queued delivery.
  • Cohort disposal already skips those three states.
  • Delegated completion notices count their whole cohort ("2 of 3 delegated tasks finished"), so they load the cohort by parent run instead of using the filtered list.
  • Measured read-only on the real thread: 154 MB in 390–460 ms → 0 rows in about 105 ms.
  • Reading the delivery state from the JSON still parsed every prompt (83 ms). Migration 075 adds a completion_delivery_state column, backfilled from the payload. An appended column sits after the payload's overflow pages, so a covering index carries the whole filter. On a copy of that data: 83 ms → 45 ms with the column alone → 3.8 ms with the index. The backfill took 0.85 s.

PR sync sweep N+1. Each group re-resolved the project list (getShell + listShells + remote hosts), about 220 SQLite reads per 39-group sweep. The most common slow statement on the new builds was this project shell select (717 runs over 50 ms). PullRequestService.withSharedProjectReads makes one sweep share one read per filter. The snapshot is closed when the sweep ends, so fibers forked during the sweep fall back to fresh reads afterwards.

No-op spans. completeProviderRetry (Codex and Claude) ran as a traced function on every model frame, which was 7% of server spans. Only a pending retry opens a span now.

Usage-limit recovery polling. The worker queried SQLite on every 5 s scheduler tick. It now uses the due hint from #27098, moved to scheduling/DueHint.ts. It runs again at the next armed reset or snooze end (new getLimitRecoveryWakeAt), on events that can change recovery, on a settings change, or after 10 minutes at most.

Tests:

  • The settled-records fixture covers each delivery state; it caught a NULL handling bug before landing.
  • A routing test asserts shared project reads inside and outside a sweep.
  • A worker test asserts idle ticks skip SQL but the wake time and a relevant event still trigger a sweep; it fails without the gate.
  • The getLimitRecoveryWakeAt assertions extend the existing limit-recovery test.
  • The settled-records fixture also covers except, nodeIds, turnItemNodeIds and getSubagentCount.
  • The delivery-preservation test asserts a task stays settled after an update without a delivery state; it fails if the upsert drops the column.
  • A delegated-delivery test asserts the notice still counts a sibling delivered earlier.
  • The MCP, delegated-task, runtime-layer and replay-fixture suites pass (643 tests).

Delegation and MCP task tools. The other heap-spike suspects read every subagent directly: delegating, remote completion, follow-ups, wake policy, and the MCP thread loader that every thread-scoped tool call goes through. Remote completion also loaded every node and every subagent turn item; on that thread the turn items are another 155 MB.

  • New task titles count subagents in SQL (getSubagentCount).
  • Handlers that act on one task read it by id, or keep it while skipping settled tasks when delivery planning needs the undelivered ones (omitSettledSubagents: { except }).
  • Projection reads gain nodeIds and turnItemNodeIds filters. Every subagent turn item's node_id is its subagent id; I checked all 4,711 on nucbox-1.
  • MCP loadProjection no longer reads subagents. Cancel, thread read and wait load the tasks they name.

Walkthrough

The server now narrows delegated-task reads, stores and filters completion-delivery state, and schedules limit-recovery lookups using wake hints. Pull-request sweeps share workspace-project reads within a scope. Claude and Codex retry-completion spans apply to event emission.

Changes: Delegated-task projection reads

Layer / File(s)Summary
Projection filters and delivery state
apps/server/src/orchestration-v2/ProjectionStore.ts, apps/server/src/orchestration-v2/ProjectionStore.test.ts, apps/server/src/persistence/Migrations.ts, apps/server/src/persistence/Migrations/075_SubagentDeliveryState.ts, apps/server/src/persistence/reconcileV2PreviewMigration.test.ts, apps/server/src/orchestration-v2/ProviderTurnControlService.test.ts, apps/server/src/orchestration-v2/DelegatedCompletionDelivery.test.ts
Projection reads support node and turn-item filters and optional settled-subagent omission. Subagent records store completion-delivery state. Migration 75 adds and backfills the state. Tests cover filtering and delivery-notification counts.
Targeted delegated-task operations
apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/mcp/OrchestratorMcpService.ts
Delegated-task operations load selected records instead of full subagent collections where applicable. Task ordinals use the projection store’s subagent count. Completion-notification paths load the full delegated-task cohort when required.

Limit-recovery scheduling

Layer / File(s)Summary
Shared due-hint factory
apps/server/src/scheduling/DueHint.ts, apps/server/src/scheduledTasks/ScheduledTaskService.ts
The scheduling module provides a reusable due-hint factory. The scheduled-task service uses it for scheduled tasks and pending webhooks.
Projection recovery wake time
apps/server/src/orchestration-v2/ProjectionStore.ts, apps/server/src/orchestration-v2/ProjectionStore.test.ts
The projection store returns the earliest future reset or snooze time for eligible threads. Tests cover reset deadlines, snooze expiry, and disabled auto-resume.
Recovery worker polling
apps/server/src/orchestration-v2/UsageLimitRecoveryWorker.ts, apps/server/src/orchestration-v2/UsageLimitRecoveryWorker.test.ts
The worker invalidates its due hint for selected events and settings changes. It skips candidate lookup while idle and records a wake time when no command was attempted. Tests check query timing.

Shared project reads

Layer / File(s)Summary
Scoped project-read sharing
apps/server/src/pullRequest/PullRequestService.ts, apps/server/src/pullRequest/PullRequestService.test.ts, apps/server/src/orchestration-v2/PullRequestSyncReactor.ts, apps/server/src/orchestration-v2/PullRequestSyncReactor.test.ts
withSharedProjectReads caches matching project reads for the scope of an effect. The sync worker wraps each sweep in this operation. Tests verify reuse within a scope and fresh reads afterward.

Retry-completion spans

Layer / File(s)Summary
Retry-completion span emission
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts
Both adapters use untraced retry-completion functions and apply named spans when emitting completed retry items.

Add focused coverage for the migration 75 backfill.

The upgrade fixture does not seed a pre-migration subagent row with completionDelivery.state, so it does not exercise the backfill. If migration 75 omits the update, historical settled rows retain NULL in completion_delivery_state; the settled-subagent predicate then leaves them visible because COALESCE(..., '') is not one of the settled delivery states.

Extend the existing upgrade test with a pre-migration row and assert the backfilled column. Include the settled-subagent read assertion only if no lower-layer migration test already covers that predicate.

Make loadTasks explicitly share its caller’s span.

loadTasks performs reusable I/O and is called by already-traced MCP operations. Use Effect.fnUntraced with a comment that explains the shared span. An unwrapped arrow does not express the documented exception.

Do not retain failed project reads for the whole sweep.

Effect.cached retains failures as well as successes. (effect.website) If one project read fails transiently, every later PR group with that filter fails without retrying, delaying their sync until another sweep. Evict failed reads while continuing to share successful and in-flight reads.
