Problem

A user reported that Claude takes hours in T3 Code on tasks that take minutes in the native Claude Code CLI. We traced three causes, all specific to T3:

  • Ultracode stayed on. Picking Ultracode once saved it as the model's default on web and mobile. Every later thread started with it, and same-model delegate_task children copied it. The Claude Agent SDK documents Ultracode as session-scoped: "interactive toggles never persist it". It adds standing multi-agent workflow orchestration. In our transcripts, sessions at xhigh/ultracode had a median API time of 10,290 s, against 309 s at high, with about 8x more output tokens.
  • Every Claude send and steer walked every skill folder (user, project, and plugin roots) before the query opened. Traces: p50 3–4 s, max 20 s per message. The result is only used when the message names a skill.
  • A finished turn waited for a display-only git and workspace refresh before finalization completed. Outbox ordering held the thread's next turn behind it. Traces: refresh p50 5.9 s, max 19 s. This affects every provider.

Change

  • Remembered model defaults and inherited delegate options turn effort: ultracode into xhigh. The current thread keeps Ultracode. Old saved defaults are cleaned on load. The helper withoutSessionScopedOptions lives once in @t3tools/shared/model and is used by web, mobile, and the MCP delegation path. A mobile follow-up keeps the options picked in the settings sheet when the user presses Save.
  • Claude skips skill discovery when a message has no attachments and no $skill mention. Attachment paths become part of the dispatched text, so messages with attachments still run discovery.
  • RunFinalizationService.finalize runs the display refresh in a service-scoped FiberSet instead of awaiting it. A per-cwd lock keeps refreshes for one checkout in order, so an older status read cannot publish after a newer one. The checkpoint is still durable before finalization completes.

No change to tokens, prompts, or model quality. The Ultracode change lowers spend for carried-over threads.

Scope and approval

These are focused fixes for measured slowdowns that users can see, and they are in established behavior. Each fix changes one call path and adds no new setting or feature. Opened at the maintainer's request.

Verification

  • Each fix has a focused test that fails on the old code and passes with the fix:
    • ClaudeAdapterV2.test.ts, "skips skill discovery for a prompt that names no skill": on the old code the skill root was read once for a plain prompt.
    • RunFinalizationService.test.ts: finalize now completes while the refresh never ends, and a second finalize does not start a second refresh for the same checkout. On the old code the test timed out. Without the lock it fails.
    • composerDraftStore.test.ts: the thread keeps Ultracode, and the remembered default is xhigh.
    • OrchestratorMcpService.test.ts: a new "session-scoped ultracode" row in the existing inheritance table.
  • vp run check:changed passed after each commit and again after the rebase: format, lint, knip, typecheck of 31 packages, and tests in 8 files.
  • CodeRabbit found the refresh overlap, which is fixed in the last commit. Its mobile finding was answered as intended behavior, and CodeRabbit agreed.
  • Reviewed with sol-loop: 2 rounds with gpt-6.1-sol. Round 1 found the mobile Save regression, which is fixed in the last commit. Sol verdict: approved.
  • Not checked: no manual run in a real client. The mobile Save paths are in screen components that have no test harness. Sol accepted that gap.

Created with Claude Opus 5.5 in Claude Code (T3 Code). Investigation and review used GPT-6.1 Sol, GPT-6 Astra, and Claude Opus 5.5 subagents.

Walkthrough

The changes normalize remembered and inherited model options, condition Claude skill discovery on message content, and schedule serialized workspace refreshes without waiting for completion.

Changes: Model option handling

Layer / File(s)Summary
Normalize remembered model options
packages/shared/src/model.ts, apps/mobile/src/state/*, apps/web/src/composerDraftStore.ts, apps/web/src/composerDraftStore.test.ts
A shared helper converts effort: ultracode to effort: xhigh. Mobile and web draft state filter session-scoped options from sticky selections and remembered options. The web test checks that the thread retains ultracode while sticky options store xhigh.
Apply explicit and inherited options
apps/mobile/src/features/threads/ThreadSettingsSheet.tsx, apps/mobile/src/features/threads/new-task-flow-provider.tsx, apps/server/src/mcp/*, apps/server/src/peer/RemoteDelegation*
Mobile updates use supplied options directly and apply remembered options only when options are omitted. MCP launches, project launches, and remote delegation filter inherited session-scoped options. Explicit options retain precedence. Tests cover inherited ultracode becoming xhigh and explicit options retaining ultracode.

Claude skill discovery

Layer / File(s)Summary
Gate skill discovery by message content
apps/server/src/provider/Drivers/ClaudeSkillDispatch.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The adapter skips discovery when a message has no attachments and does not mention a skill. Otherwise, it discovers skills and retains enabled, user-invocable skills. A test verifies that a message without a skill mention does not cause the skills root to be read.

Run finalization refresh

Layer / File(s)Summary
Run refresh effects without awaiting completion
apps/server/src/orchestration-v2/RunFinalizationService.ts, apps/server/src/orchestration-v2/RunFinalizationService.test.ts
Finalization schedules post-checkpoint workspace refresh effects through a FiberSet and serializes refreshes by cwd. The test checks that finalization proceeds while the first refresh remains pending.

Estimated code review effort: 3 (Moderate) | ~25 minutes

Preserve pending Ultracode edits within the settings sheet.

If a user selects Ultracode for a pending model, switches to another model, then returns before saving, the sheet restores xhigh from this memory. Save then applies xhigh instead of Ultracode. Keep pending options separately from normalized cross-thread memory. (raw.githubusercontent.com)

Not changing this. It is intended. Ultracode is session-scoped, so remembered options never hold it. Switching to another model and back is a fresh pick that restores remembered options. Web does the same when you switch models in the composer (useComposerSelection reads stickyOptionsByModelByProvider, which now stores xhigh). Keeping a second per-model store for unsaved sheet edits would add new state for a rare path. The user can pick Ultracode again before Save, and the current thread keeps it once saved.

@Bil0000 Understood. Switching models is a fresh pick that restores remembered options, so restoring xhigh is intended. My finding incorrectly assumed that unsaved Ultracode selections must survive model switches. I withdraw the finding. No change is needed.

5Learning: Within repository "pingdotgg/t3code", only for files matching one of ["apps/mobile/src/state/use-model-option-memory.ts"]: When reviewing remembered model options in apps/mobile/src/state/use-model-option-memory.ts, preserve normalization of session-scoped Ultracode to xhigh. Treat switching away from a model and back as a fresh pick that restores remembered options. Do not require a separate pending-options store solely to retain unsaved Ultracode across model switches; this does not require normalizing the current thread's explicitly saved Ultracode selection.

574# Cache
575
576In many applications, handling overlapping work is common. For example, in services that process incoming requests, it’s important to avoid redundant work like handling the same request multiple times. The Cache module helps improve performance by preventing duplicate work.
577
578Key Features of Cache:
579
580Feature | Description
581--- | ---
582Compositionality | Allows overlapping work across different parts of the application while preserving compositional programming.
583Unified Sync and Async Caches | Integrates both synchronous and asynchronous caches through a unified lookup function that computes values either way.
584Effect Integration | Works natively with the Effect library, supporting concurrent lookups, failure handling, and interruption.
585Cache Inspection | Provides functions like `Cache.size`, `Cache.keys`, and `Cache.entries` to inspect the current state of the cache.
586
587## Creating a Cache
588
589A cache is defined by a lookup function that computes the value for a given key if it’s not already cached:
590
591 `
592
593 type Lookup<Key, Value, Error, Requirements> = (
594
595 key: Key,
596
597 ) => Effect<Value, Error, Requirements>
598
599 `
600
601The lookup function takes a `Key` and returns an `Effect`, which describes how to compute the value (`Value`). This `Effect` may require an environment (`Requirements`), can fail with an `Error`, and succeed with a `Value`. Since it returns an `Effect`, it can handle both synchronous and asynchronous workflows.
602
603You create a cache by providing a lookup function along with a maximum size and a time-to-live (TTL) for cached values.
604
605You create a cache by providing a lookup function along with a maximum size and a time-to-live (TTL) for cached values.
606
607 `
608
609 declare const make: <Key, Value, Error, Requirements>(options: {
610
611 readonly capacity: number
612
613 readonly timeToLive: Duration.Input
614
615 readonly lookup: Lookup<Key, Value, Error, Requirements>
616
617 }) => Effect<Cache<Key, Value, Error>, never, Requirements>
618
619 `
620
621Once a cache is created, the most idiomatic way to work with it is `Cache.get`. `Cache.get` returns the current value in the cache if it exists, or computes a new value, puts it in the cache, and returns it.
622
623If multiple concurrent processes request the same value, it will only be computed once. All other processes will receive the computed value as soon as it is available. This is managed using Effect’s fiber-based concurrency model without blocking the underlying thread.
624
625Example (Concurrent Cache Lookups)
626
627In this example, we call `timeConsumingEffect` three times concurrently with the same key. The cache runs this effect only once, so concurrent lookups will wait until the value is available:
628
629 `
630

807## Concurrent Access
808
809The cache is designed to be safe for concurrent access and efficient under concurrent conditions. If two concurrent processes request the same value and it is not in the cache, the value will be computed once and provided to both processes as soon as it is available. Concurrent processes will wait for the value without blocking the underlying thread.
810
811If the lookup function fails or is interrupted, the error will be propagated to all concurrent processes waiting for the value. Failures are cached to prevent repeated computation of the same failed value. If interrupted, the key will be removed from the cache, so subsequent calls will attempt to compute the value again.
812
813## Capacity
814
815A cache is created with a specified capacity. When the cache reaches capacity, the least recently accessed values will be removed first. The cache size may slightly exceed the specified capacity between operations.
816
817## Time To Live (TTL)
818
819A cache can also have a specified time to live (TTL). Values older than the TTL will not be returned. The age is calculated from when the value was loaded into the cache.
820
821## Functions
822
823In addition to `Cache.get`, the `Cache` module provides several other functions for working with a cache. Each takes the cache as its first argument, for example `Cache.refresh(cache, key)`:
824
825Function | Description
826--- | ---
827`Cache.refresh` | Triggers a recomputation of the value for a key without removing the old value, allowing continued access.
828`Cache.size` | Returns the current size of the cache. The size is approximate under concurrent conditions.
829`Cache.has` | Checks if a value associated with a specified key exists in the cache. Under concurrent access, the result is valid as of the check time but may change immediately after.
830`Cache.invalidate` | Evicts the value associated with a specific key.
831`Cache.invalidateAll` | Evicts all values from the cache.</excerpt>

837 * Failures are stored — interruptions are not. With `Effect.cached` or a fixed-TTL `cachedWithTTL`, a transient `HrisUnavailable` is replayed to every caller until expiry, which for `cached` is never. Branch on the `Exit` and return `0` for non-success so the next caller retries. Interrupted computations are never cached: the pending work is interrupted only after every waiting caller has been interrupted (abandonment), and the next caller then starts a fresh computation.
838 * `Cache` applies the same abandonment rule: an interrupted `Cache.get` lookup is never retained; the next caller starts fresh.
839 * Reach for `Cache` as soon as there is a key, and for `Resource` when the value needs scheduled refresh or owns a resource.
840
841Official guide: Caching Effects (its "once" heading is not an API name; the code under it uses `Effect.cached`).
842
843## Cache
844
845`effect/Cache` — stable
846
847Effectful memoization table with bounded capacity and optional TTL. A lookup effect is provided at construction; the cache handles concurrent requests, LRU eviction, and TTL expiry.
848Function | What it does
849--- | ---
850`Cache.make({ lookup, capacity, timeToLive? })` | Create a cache. `timeToLive` is a `Duration.Input` string or millis; omitted means entries never expire.
851`Cache.makeWith(lookup, { capacity, timeToLive: (exit, key) => Duration })` | Dynamic TTL — vary per key or per outcome. A zero TTL means "share the in-flight lookup, retain nothing afterwards".
852`Cache.get(cache, key)` | Get or compute. Concurrent misses share one lookup.
853`Cache.getOption(cache, key)` | Read without starting a lookup. Returns `None` if absent/expired, awaits an existing pending entry, returns `Some<A>` on success, and fails with the cached lookup error on failure.
854`Cache.getSuccess(cache, key)` | Non-blocking peek: `Some<A>` only for an entry that has already resolved successfully; `None` for missing, expired, pending, or failed entries. Never fails.
855`Cache.refresh(cache, key)` | Force and await a new lookup. An existing entry remains readable by other fibers until it completes, then is replaced. Unlike concurrent `get` misses, concurrent `refresh` calls are not deduplicated.
856`Cache.invalidate(cache, key)` | Remove one entry, whatever its outcome.
857`Cache.invalidateWhen(cache, key, predicate)` | Remove the entry only if it holds a successful value matching the predicate; returns whether it did. A cached failure is never removed this way.
858`Cache.invalidateAll(cache)` | Clear everything.
859`Cache.set(cache, key, value)` | Manually populate an entry (useful for seeding).
860`Cache.has(cache, key)` | Check for an unexpired entry without lookup.
861`Cache.size` / `keys` / `values` / `entries` | Inspect the table. Expiry is lazy: `size` counts stored entries, including ones whose TTL has passed but that nothing has touched yet, while `keys`, `values`, and `entries` skip (and drop) expired ones; `values` and `entries` list successful entries only.
862
863> Tip: By default, services needed by `lookup` are captured at construction time. Pass `requireServicesAt: "lookup"` to instead capture them at call time — useful when the lookup needs request-scoped services that weren't available when the cache was built.
864Short (seconds) | Callers share one failure briefly, then one of them retries | The usual answer: it shields a struggling backend from a stampede and still recovers quickly
865Zero | Concurrent callers share the in-flight lookup; the next caller starts a new one | The lookup is cheap, or an outer retry policy already paces the calls
866
867TTL is a deadline; invalidation is an event. When something tells you an entry is wrong now — a comp-band update arrived, a retry path just saw a transient failure — call `Cache.invalidate` (any outcome), `Cache.invalidateWhen` (successful values only), or `Cache.refresh` (replace while readers keep the old value). Do not shorten the TTL to approximate an event you could have handled.
868
869> Correctness guarantees. `invalidateWhen` never deletes a replacement entry that was written while it was reading the old value; an interrupted or zero-TTL `refresh` of a missing key never removes a newer value written by `Cache.set`; `refresh` never pushes the table past `capacity` when its key is evicted mid-refresh; and a synchronously interrupted lookup is never retained. A cache hit also re-stores the entry under the key instance you just passed to `get`/`getOption`, not the instance used to create it, so replacing an equal-but-distinct key object (for example, a rebuilt `Data.Class`) does not keep the old object pinned in the map. The same guarantees apply to `ScopedCache`.
870 yield* ScopedCache.invalidate(cache, "us-west-2")
871 })
872 )
873 // When the outer Effect.scoped closes, any remaining shard connections are released.`
874
875> Warning: If your value has no resources to release, use `Cache` instead. `ScopedCache` runs scope machinery on every entry. Reach for it only when your lookup calls `Effect.acquireRelease`, opens a socket, or otherwise needs cleanup on eviction.
876
877`ScopedCache.makeWith` accepts the same `(exit, key) => Duration` policy as `Cache.makeWith`, and the key and failure TTL guidance applies unchanged. Two lifetime details are specific to it: `ScopedCache.invalidateAll` detaches every entry from the table before closing the entry scopes (concurrently), so an entry that a finalizer re-creates is kept and released later instead of being discarded unreleased; and after the cache's own scope has closed, `get`, `set`, `refresh`, and the invalidation operations are interrupted rather than reopening anything. A shared lookup also survives any single caller's interruption — if every waiter leaves while the lookup is still pending, the lookup itself is interrupted and its scope closed, and missing-key lookups (including `refresh`) run in a daemon fiber so one caller's cancellation never corrupts the entry for the others.
878
879When to use: the cached value holds a resource (live connection, file handle, in-process child) that must be released precisely on expiry or eviction.

Serialize local-status refreshes by cwd.

FiberSet.run forks observer.refresh(...), so consecutive finalizations can overlap. Each refresh invalidates the local-status cache, reads workflow.localStatus, and unconditionally updates and publishes the broadcaster cache. Effect 4.0.5 removes an invalidated entry without interrupting an in-flight lookup. An older read can therefore finish after a newer read and publish stale status.

Serialize or coalesce these background refreshes by cwd, and add a controlled overlapping-refresh test.

✅ Addressed in commit e4f1339

Valid. Fixed in e4f1339. Finalize refreshes now run under a per-cwd KeyedLock inside the forked fiber. Refreshes for one checkout run in order, and finalize still does not wait. The existing finalize test now runs two finalizes with a refresh that never ends. It asserts that the second refresh does not start. Without the lock it fails, with 4 entries instead of 3.

@Bil0000 The supplied code puts the per-cwd lock inside the forked fiber. This prevents overlapping finalization refreshes for one checkout without blocking finalize. The test you described covers both requirements. This addresses the finding.

Assert that the queued refresh eventually runs.

This assertion proves that finalize returns while the first refresh is pending. It also passes if the second refresh is dropped or never starts. Release the first refresh with a Deferred, await a second-refresh Deferred, and assert that both refreshes execute in order. (raw.githubusercontent.com)

✅ Addressed in commit 50fb2be

Valid. Fixed in 50fb2be. The test now releases the first refresh with a Deferred, then waits for a second Deferred that only the second refresh completes. It asserts that both refreshes ran in order. The test still fails without the lock.

@Bil0000 The supplied test now releases the first refresh, waits for secondRan, and checks the final refresh order. This addresses the missing queued-refresh assertion while preserving the checkpoint-order check.

Add a failure-then-success refresh test.

The replacement test covers only successful refreshes. It will not detect a regression where a RunFinalizationRefreshError fails to release the per-checkout lock and blocks a later same-checkout refresh. Add a focused case that fails the first background refresh, then asserts both checkpoints and the later successful refresh.

@coderabbitai Skipping the failure-then-success nitpick on RunFinalizationService.test.ts. The lock comes from @t3tools/shared/KeyedLock, which releases through Effect.acquireUseRelease. packages/shared/src/KeyedLock.test.ts ("keeps no lock for a key once its holders and waiters are gone") already proves that a failing holder frees its key. Our testing rule is to test a shared helper or its caller, not both, so a second copy of that case here would only repeat it.

@Bil0000 Agreed. I checked the implementation and the cited test. KeyedLock.withLock uses Effect.acquireUseRelease, and the test asserts that no active key remains after a holder fails. RunFinalizationService uses that shared lock directly.

The additional failure-then-success case would duplicate the shared helper’s coverage. I withdraw that test request. No change is needed for this comment.

merged current main without rebasing at 948dff5ae7c19c95f4c907e2d026f8e543795ece. the conflict resolution preserves main's unsupported-option filtering alongside this pr's inherited ultracode normalization. explicit choices remain intact. the diff stays at 18 files.

both independent code reviewers approved this head without findings.

blacksmith vp run check:changed ce48e459001817092f45f70f334212bb2b973288 passed: formatting, lint, knip, graphql, all 33 affected package typechecks, 12 focused test files, and react compiler checks. the worker's shallow merge boundary was repaired before this run. no check failures were suppressed.

live claude provider behavior and native mobile remain unverified; claude is unauthenticated. the existing runtime evidence remains on the verification comment. macroscope's human decision about remembered and inherited defaults remains for maria.

Verified in a real dev environment

verified the shared finalization change as-is at 1550c23a921e9212f5a24f31bb7c7433d767f15e. this is partial runtime coverage; the claude-specific paths remain unverified. no source changes or pushes were needed.

  • installed and ran github/main (e4ecaf629d) in a separate worktree first, stopped it, then installed and ran the exact pr head with fresh isolated state. only one dev server ran at a time.
  • drove the web app with repository playwright and /usr/bin/google-chrome. added a real git repository through the app and sent real codex gpt-6.1-sol turns through the local proxy. no mocks, fixtures, replay provider, or demo data.
  • main traces reproduced the blocking order: finalization took 280.155 ms, including a 138.859 ms display refresh. on the matching pr turn, finalization took 142.481 ms and refresh continued for another 174.037 ms after finalization returned. subsequent real turns showed the same independent refresh order. this proves the changed ordering; it is not a task-speed benchmark. the small repo did not reproduce the reported multi-second stalls.
  • neighboring behavior passed: file edits and readback, a follow-up turn, ctrl-click to queue during a running turn, automatic execution of the queued turn, durable per-turn checkpoint diffs, and reload. the final file and latest diff show second-turn followed by queued-turn. main's steer interaction also worked.
  • vp run check:changed fa4e4677c0ecc437aece2dcea39f76ef31414152 passed through blacksmith-check: format, lint, knip, graphql, 31 package typechecks, 508 tests in 11 files, and react compiler coverage. the worker needed the public pr history fetched and an explicit base because its snapshot lacked origin/main history. all 32 reported pr ci checks were successful when inspected.

real queued turns, with startup/reload blank frames removed:

pr: queueing and completing two real codex turns

pr: latest checkpoint preserves both real file edits

Not verified

  • claude skill discovery, skill/attachment dispatch, ultracode selection, persisted-default migration, and inherited/explicit ultracode delegation through a real provider session. claude is installed but unauthenticated, so the app disables its composer entry. passing focused tests do not replace missing runtime coverage.
  • native ios/android settings-sheet save behavior, native electron, macos/windows, remote delegation, relay/tunnel connections, and long-running/failing display refresh contention. this pass used linux web and local codex.
  • no end-to-end task-speed improvement is claimed. the traces establish that refresh no longer blocks finalization, not the original report's speedup.

Test explicit options against conflicting remembered options.

When remembered options differ from options, this branch must keep the explicit values. Add a focused case to new-task-flow-provider.test.tsx that remembers one value and selects another. That case would catch the overwrite this change fixes. The supplied test context shows no assertion for this branch.

As per coding guidelines, “Logic, data, state, and protocol bugs get a failing test first (the fewest that fail on main for that reason, in an existing case of the module's test file where one fits), then the fix.”

Add an attachment-only skill-dispatch case.

The existing test includes an attachment, but its skill mentions are all in input.message.text. Add an attachment name such as requirements $review file.pdf with a message that contains no skill mention. Assert that the offered message contains /review.
