An OpenCode 1 session's permission rules were set only at session.create (and on rollback forks). A thread created in Full access and later switched to Supervised kept *: allow, so OpenCode never asked for approval even though the UI said approvals were required.

resumeThread and startTurn now recompute openCodePermissionRules for the current runtime policy and call session.update when the rules differ from the last ones applied to that session. The adapter tracks the applied rules per session, the same way the OpenCode 2 adapter does with rulesFor/sameRules. A new adapter test covers resume (no-op when the rules are unchanged, update when they change) and turn start. The test fails on main.

Approvability

Verdict: Approved at f4599f4

Macroscope's review found this PR approvable — Existing OpenCode sessions now receive the selected runtime permissions on resume and before subsequent turns. The change reuses existing permission rules without changing defaults and avoids redundant session updates.

Walkthrough

OpenCode thread state now tracks native permission rules. The adapter derives rules from runtime policy during thread creation, applies supplied policies on resumption, and applies policies before turn startup. It skips native session updates when the ordered rules are unchanged.

Changes: OpenCode runtime permissions

Layer / File(s)Summary
Track and apply native permission rules
packages/provider-opencode/src/server/adapter.ts
Thread state records native permissions. Registration initializes or refreshes them. The adapter updates the native session only when ordered rules differ, and records rules installed during child-thread and new-thread creation.
Apply permissions on resume and turn startup
packages/provider-opencode/src/server/adapter.ts, packages/provider-opencode/src/server/adapter.test.ts
Resumption applies the runtime policy when one is supplied. Turn startup applies its policy before continuing. Tests verify that unchanged rules do not trigger an update and changed rules do.

Switching a thread from Full access to Supervised may not apply to child sessions that already exist, and the ensureThread path for existing threads does not refresh permissions. Address the child-session case before merge; the ensureThread gap is a small fix.

Forward the policy when ensureThread resumes an existing thread.

ensureThread receives a required runtimePolicy, but this branch passes only providerThread to resumeThread. The new policy check therefore cannot update permissions on this path. An existing Full-access session remains Full-access after ensureThread returns with a Supervised policy; only a later startTurn repairs it. Pass threadInput.runtimePolicy to resumeThread. 89### Description
90
91When a parent session has an explicit `external_directory` allow rule (set via SDK `session.create({ permission })` or via the session permission API), spawning a subtask through the task tool creates a child session whose permission ruleset does not contain the inherited `external_directory` allow. The subagent then asks the user for permission to read files in a directory the parent had already approved.
92
93Root cause is in `packages/opencode/src/session/prompt.ts` around line 1228. The task tool calls `prompt()` for the child session with `tools: { task: false, todowrite: false }`. `SessionPrompt.prompt` converts those into per-call permission rules and writes them to `session.permission` via `sessions.setPermission`, replacing whatever was there instead of merging. The child session is created moments earlier by `sessions.create` with the derived rules from `deriveSubagentSessionPermission` (including any inherited `external_directory` allows), and the prompt-time write then clobbers them.
94
95The fix is one line: merge the per-call tool rules into the existing `session.permission` instead of replacing it.

105### Steps to reproduce
106
107 1. Create a parent session via SDK with `permission: [{ permission: "external_directory", pattern: "/tmp/foo/*", action: "allow" }]`.
108 2. Send a prompt with a `subtask` part targeting a subagent (e.g. `explore`).
109 3. The subagent attempts to read `/tmp/foo/secret.txt`.
110 4. A pending `external_directory` permission request is emitted instead of the read succeeding.
111
112After the fix the child session permission contains the inherited `external_directory` allow and the subagent reads the file directly.

165### Description
166
167When the task tool creates a subagent session it does not store the agent name on the new session row. If that subagent spawns its own subagent, the permission resolver reads `parent.agent` to inherit the parent's restrictions, finds it empty, and drops the parent agent's edit denies on the grandchild.
168
169The practical effect: a Plan-mode subagent that spawns another subagent ends up with a grandchild that can edit files, even though Plan mode forbids edits.
170
171This is a different root cause from #30527, which is about prompt-time `setPermission` replacing instead of merging the ruleset. Here the `agent` column is never written at session creation, so `deriveSubagentSessionPermission` has nothing to read. Same code area as #26597, #26700, and #26747.
172
173I have a fix that sets the agent name when the subagent session is created.

246### Commands
247
248After `session/new`, `session/load`, `session/resume`, or `session/fork`, OpenCode sends `available_commands_update` with the directory’s commands and the built-in `compact` command. It sends another update when the commands change.

254
255If a prompt starts with `/` and the name of an available command, OpenCode runs that command with the rest of the text as arguments. OpenCode sends other text that starts with `/` as a normal prompt. `/compact` runs compaction.

257### MCP servers
258
259The client can pass stdio and HTTP MCP servers on `session/new`, `session/load`, `session/resume`, and `session/fork`. OpenCode adds them to the session’s directory for the life of the `opencode acp` process, so other sessions in that directory can use them. OpenCode passes HTTP headers through and doesn’t use OAuth for these servers. It doesn’t advertise MCP over SSE, and it doesn’t support MCP over ACP.

278When a request changes files, OpenCode includes `diff` content if it can build a preview of the change.
279
280### Child sessions
281
282While a prompt runs, OpenCode sends updates from its child sessions as `session/update` notifications on the prompt’s session. Each update carries `_meta["opencode/child-session"]`. OpenCode prefixes tool call IDs with `<childSessionId>:`. It prefixes tool call titles with the child session’s title if the child session has one.
283

285 "sessionUpdate": "tool_call",

290 }
291 }`
292
293Field | Type | Description
294--- | --- | ---
295`id` | `string` | The child session’s ID.
296`parentID` | `string` | The ID of the child session’s parent session.
297`depth` | `number` | 1 for a child of the prompt’s session, 2 for a child of that child session, and so on.
298`title` | `string` | The child session’s title. OpenCode omits it when the child session has no title.
299
300Permission requests from a child session carry the same key on `toolCall._meta`. After the prompt ends, OpenCode stops sending child session updates. Permission requests and questions from child sessions still reach the client.
301
302### Retry status
303
304When a model call fails and OpenCode schedules a retry, it sends `session_info_update` with `_meta["opencode/retry"]`. When the retry starts, OpenCode sends `"opencode/retry": null`. Clear any retry status when the prompt response arrives.
305
306 `{
307 "sessionUpdate": "session_info_update",

313 }
314 }
315 }`
316
317Field | Type | Description
318--- | --- | ---
319`attempt` | `number` | The retry attempt number.
320`nextRetryAt` | `string` | The time of the next attempt, in ISO 8601 format.
321`error` | `object` | Has `type` and `message`. Also has `status` and `response.body` if the provider sent them.
322
323### Compaction marker
324
325If the client doesn’t advertise `session.compaction`, OpenCode reports compaction progress as `session_info_update` with `_meta["opencode/compaction"]`. OpenCode always uses the marker for child session compactions.
326
327 `{
328 "sessionUpdate": "session_info_update",
329 "_meta": { "opencode/compaction": { "status": "started", "messageId": "msg_123", "reason": "auto" } }
330 }`
331
332Field | Type | Description
333--- | --- | ---
334`status` | `string` | `started`, `completed`, or `failed`. A cancelled compaction is `failed`.
335`messageId` | `string` | Pairs a compaction’s start with its end. Treat it as an opaque key.
336`reason` | `string` | `auto` or `manual`.
337`error` | `object` | Present when `status` is `failed`. Same shape as the retry `error`.
338
339The marker doesn’t include a summary. An automatic compaction can fail without a `started` marker. When the client loads a session, OpenCode replays each completed or failed compaction as one marker.</excerpt>

Update active child sessions when the runtime policy changes.

OpenCode child sessions inherit permissions when they are created. A root session.update does not refresh existing children. Update each active child session when applying a new policy. Remove stale parent-derived rules such as *: allow, while preserving child-specific restrictions such as task: deny.

The current openCodeChildPermissionRules helper preserves non-deny child rules, so it must not treat an old parent-derived *: allow rule as child-specific.

8Update the applyPermissionRules flow to apply a changed runtime policy to each

10openCodeChildPermissionRules so stale parent-derived `*: allow` rules are
11removed while child-specific restrictions such as `task: deny` remain.
