fix(server): stop PR watch flapping while workflows await approval#24599

Unproven: not run against a live fork PR on GitHub. The replay below feeds the watch the reads the issue describes.

Closes #24575

Problem

When a fork PR's workflows waited for a maintainer to approve them, the PR watch woke the agent again and again on the same commit, flipping between "Checks failed" and "All 7 checks passed". The watch counted a workflow awaiting approval as a failed check. GitHub lists those workflows in a separate read that sometimes comes back empty, and then the 7 bot checks alone looked like a pass.

Change

Now the watch sends one "waiting for approval" message and stays quiet. It remembers each waiting workflow run until a job from that run finishes, so an empty read no longer counts as a pass. When GitHub can't say whether anything awaits approval, the watch keeps its last state instead of reporting a failure. Real failures still wake the agent. A new push clears the remembered runs, which also means a run deleted before it ever starts keeps the watch waiting until the next push.

Scope and approval

This PR changes the wire protocol in packages/contracts by adding optional PullRequestCheck.placeholder and ThreadPullRequestWatch.awaitingApproval fields. Under the maintainer approval rule, it requires an approving GitHub review from Theo (t3dotgg) or Julius (juliusmarminge) before merging. Neither approval is currently recorded.

This addresses #24575. The provider marker, persisted watch state, evaluator, notifications, and tests all support the same approval-wait defect. No prior maintainer scope-approval comment is recorded on that issue. Macroscope requests human review of this persisted behavior change; that review remains for the maintainer's merge decision.

Verification

The issue's case (7 bot checks pass, 4 workflows await approval) replayed through the watch, one line per check:

Before, on main 5f64c5eb29:

text
1workflows await approval - Checks failed on 3b4d479:
2approval read answers empty - All 7 checks passed on 3b4d479.
3approval read returns - Checks failed on 3b4d479:
4approval read answers empty - All 7 checks passed on 3b4d479.
5maintainer approves, jobs run (no wake)
6jobs finish - All 11 checks passed on 3b4d479.

After, on this branch at 94a34cd:

text
1workflows await approval - Checks are waiting for approval or action from a maintainer on 3b4d479:
2approval read answers empty (no wake)
3approval read returns (no wake)
4approval read answers empty (no wake)
5maintainer approves, jobs run (no wake)
6jobs finish - All 11 checks passed on 3b4d479.

Tested on corex with Node 26.12.1: the three new tests in pullRequestWatch.test.ts pass at d07d7bf, and all three fail with main's pullRequestWatch.ts. vp run check:changed passes at 7bb6643 after merging main.

GitHub CI run 40232179643 passed at d07d7bf987b103536e24d8778a04a8972b5f1596, including lint, all eight server test shards, build and typecheck, shared tests, web tests, and release smoke. The focused evaluator suite also passed all 23 tests on corex with Node 26.12.1 and Vitest 5.0.5. Full desktop interaction and full process restart/event replay were not verified in that run.

Current head f452f47dcf5e830fcb1c09f42b94a5006efdd235 includes main at 53896aea76 and adds regression coverage for recovery from failures or pending checks beside unread approvals, plus approval persistence across reactor restart and rewatch. GitHub CI run 40260213024 passed at this exact head, including lint, all eight server test shards, build and typecheck, shared tests, web tests, and release smoke. CodeRabbit completed its review of this head with no blocking findings.

Maria's independent verification ran exact head f452f47dcf5e830fcb1c09f42b94a5006efdd235 in an isolated Linux dev environment with Chrome and real Codex turns. She confirmed one successful-check notification, stop/rewatch, and server restart recovery without a duplicate notification. Eight existing approval/workflow and recovery tests passed. This verifies the normal successful-watch path. The same-repository PR had no approval-gated workflows, so live approval waiting, intermittent missing approval reads, and approval-to-completion remain unverified. No source changes were made.

Fixed in 5f30a39eaf, included in 4304491266. An unread approval response now prevents confirming a pass, while an observed failed or pending gate check, or any real approval, clears the previous pass. Red-first coverage verifies failure and pending recovery each wake once, real approvals block passing, and positive-only unread reads preserve owed checks. The evaluator transcript confirms failure plus placeholder clears passed, then recovery emits checks-passed. Final-commit Boat job j_tw4hethmfjxipsdlpf7a passed check:changed with 161 tests; independent review found no code blockers.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval β€” This fixes PR-watch flapping by changing action-required checks from failures into persisted approval waits, including behavior across GitHub reads, orchestration state, and agent notifications. The cross-layer state and changed semantics for existing watches warrant human review.

Explanation

Issue #24575 requirements are met. evaluatePullRequestWatch excludes action-required checks from failures and tracks approval state through persisted awaitingApproval values. It emits one approval-wait change, suppresses passes while approval remains unresolved, retains workflow-run state across missing reads, and clears it when the head changes or a job finishes. Placeholder checks block a confirmed pass when approval status is unknown. Tests cover missed reads, persistence and restart, head changes, completion, unknown status, and real failures.

Explanation

The changes remain within issue #24575. The modified watch evaluator, provider placeholder marker, persisted contracts, reactor and orchestrator state handling, user messages, documentation, and regression tests directly support approval-wait behavior. No unrelated change is demonstrated.

Keep the failedChecks and no-checks timer when a read has only the approval placeholder.

A read whose only check is the unread approval placeholder counts as "checks seen" in the evaluator. Two effects follow:

  • The if (detail.checks.length > 0) block sets failedChecks to the failed real checks, which is []. The next read with failing checks then reports checks-failed again, which is the flapping this PR is meant to stop.
  • checksAwaitedSince becomes null, so the no-checks timer restarts instead of continuing.

The evaluator already treats an empty check list as "keep the last state." A placeholder-only read should follow the same rule. Count only real checks for these two updates:

diff
1- checksAwaitedSince = detail.checks.length > 0 ? null : awaitedBefore;
2+ checksAwaitedSince = real.length > 0 ? null : awaitedBefore;

The pass gate already skips every read that has a placeholder (!unread), so it needs no change.

[gpt-6.1-sol][Codex] on behalf of shivam.

Fixed in 4304491266. Placeholder checks are excluded from the checks-seen and gate logic. A placeholder-only read now preserves failures, name-based approvals and the no-checks timer like an empty read. Two red-first regressions verify preserved state without repeated failure or approval wakes, confirmed-head reset, and exactly one no-checks notice. All 31 evaluator tests pass; final-commit Boat job j_tw4hethmfjxipsdlpf7a passed check:changed with 161 tests. Independent review found no code blockers.

@shivamhwp Thanks. The supplied code addresses this finding: placeholder-only reads preserve check state and keep the no-checks timer running. A confirmed head change still resets check state. I have not independently run the reported tests.

Preserve unread check state in a mixed partial read.

If a read contains a real check and a placeholder for unavailable checks, this branch replaces failedChecks with only the failures in that read. It also drops a previously awaited approval stored by check name when that check is absent. When either check reappears, the watch can send the same failure or approval wake again. Keep state for checks the placeholder leaves unread, and clear it only when a later read identifies their new status. Add a regression test with an unrelated real check beside the placeholder.

[gpt-6.1-sol][Codex] on behalf of shivam.

Fixed in 52e57299f7. A mixed response with an unread placeholder now retains absent failure and name-based approval entries, while listed checks still update or clear their own state. Complete reads still drop absent name keys, and a new head resets them. The regression failed on 43044912 and now passes; the evaluator transcript shows no duplicate wake after the missing checks return. Independent review passed 105 focused tests plus the persistence test. Boat job j_2h3dgypdwajcg3ckuzvu passed check:changed on this commit, including 162 tests and 30 package typechecks.

@shivamhwp The supplied code addresses this finding. missed preserves absent failure and name-based approval entries during mixed placeholder reads. Listed checks still update their own state, complete reads clear absent name-based entries, and a new head resets the state.

Your reported regression results cover the duplicate-wake case. I did not rerun the tests.

Exercise approval-state reset on a new head.

The added test keeps the fixture on "aaaaaaaaaa". It checks one missing read, but never evaluates the waiting state after a confirmed new head. The existing head-change test clears owedChecks, not awaitingApproval. A stale approval key could therefore block checks-passed on the new head without failing these tests.

@coderabbitai Fixed in d07d7bf987. The approval regression test now pushes a new head while a run is still awaited and expects the pass. It fails if the head-change reset is removed. I added it to the existing test rather than a new one to keep the test set small.

@shivamhwp, your approval tracking and unread-response marker address the defect without changing the provider architecture. I preserved all your commits and implementation, merged github/main at 53896aea76, and added only verification: recovery after real failures/pending checks beside unread approvals, plus approval persistence through reactor restart and rewatch. The existing integration test now covers both pending and approval cases. Final diff against that main: 11 files, +185/-32.

Verification at f452f47dcf5e830fcb1c09f42b94a5006efdd235:

  • All three approval regressions fail with current main's evaluator and pass with this implementation.
  • Blacksmith vp run check:changed passes: 156 tests in eight files, 30 package typechecks, lint, format, knip, and GraphQL checks.
  • Real GitHub provider detail/check reads passed for this PR, the historical incident, and a currently approval-gated fork. That fork returned five action-required workflows.
  • Replayed the incident's seven real rollup checks and four real approval runs fetched from GitHub. Omitting the approval slice makes main alternate failed/passed; this branch emits one approval notice and then stays quiet.
  • GitHub CI is green, the PR is mergeable, both independent reviews approve this exact head, and the latest bot review has no actionable findings. Existing review threads are resolved.

Unverified: a maintainer approving workflows and the resulting completion notification through a live client; no browser captures establish that transition. Macroscope still requests human approval for the persisted behavior change. The conservative existing limit remains: deleting a remembered run before it starts can keep the watch waiting until the head changes. Mixed-placeholder preservation and legacy migration have no dedicated regression cases in the final test set.

Verified in a real dev environment

partial verification at f452f47dcf5e830fcb1c09f42b94a5006efdd235. no source changes or pushes.

  • installed and ran github/main (975b3dc568) in /home/user/main, then stopped its captured process group before starting the exact pr head. the pr used isolated /home/user/t3code/.t3 state. one dev server ran at a time.
  • used repo playwright with /usr/bin/google-chrome. created the project and threads through the web app and ran real codex gpt-6.1-sol turns through the local proxy. no injected host responses or seeded scenario data.
  • on both main and the pr, the agent registered a watch for the literal github pr url. real github reads produced one "all 8 required checks passed" notification, and the real codex agent responded in the client. the persisted pr watch had the exact head, passed: true, eight passed check names and awaitingApproval: [].
  • the stop button removed the watch. a real provider turn rewatched it with the prior passed-check state. server restart recovered the watch. the existing pass notice remained singular after restart and the next two-minute sweep.
  • blacksmith ran existing approval/workflow and recovery cases: 8 passed, 128 excluded by the name filter across three files. command: vp test run apps/server/src/orchestration-v2/pullRequestWatch.test.ts packages/source-control-github/src/server/GitHubPullRequestProvider.test.ts apps/server/src/orchestration-v2/runtimeLayer.test.ts -t "recovers|pullRequestWatch|approval|workflow". these are supplementary automated checks, not proof of a live approval cycle. github ci is green at this head.

main: normal successful-watch path, not a reproduction of approval flapping:

main receives a real github checks-passed wake

pr: real github wake, real codex reply, watch remains active:

pr receives a real github checks-passed wake and keeps watching

pr: clicking stop removes the active-watch control:

stop the active github watch in the real web client

runtime transcript: read-only persisted watch and notification output.

Not verified

the changed approval-wait behavior is not live-verified. #24599 is now a same-repository pr with passing checks and no workflows awaiting approval. the github provider only queries approval-gated workflow runs for cross-repository prs. consequently i could not reproduce the original approval flapping on main or exercise one approval notice, intermittent missing approval reads, remembered nonempty approvals across restart, approval-to-job completion, or failure recovery against real github state. the brief prohibits touching another pr, and i did not manufacture host responses or modify workflows to create a trigger. passing tests and the successful-watch path do not close this gap.

native ios/android, electron desktop, macos/windows, remote/relay/tunnel connections, and other coding providers were not exercised on this linux vm. check:changed was not run because no fix or source change was made. no merge, close, rebase, force-push, or label change was performed.

verification incident: shell quoting while writing this report unintentionally started a local test command. it was interrupted; no local test result is counted above.
