Behavior

Preserve active OpenCode 2 turns while a T3-owned server recovers. Failed reads back off up to 30 seconds. Stop settles while history is blocked, background approvals and questions stay answerable, and unread SSE subscriptions close with their connection. A deleted native session ends individually without blocking siblings; actual process exit fails the affected turns promptly. External servers retain their bounded retry policy.

Why

The old 15-second deadline covered reconciliation across every session. A slow but live server could exhaust all five attempts, fail every turn, then be idle-stopped despite still doing work. Recovery now follows the owned process's lifetime instead of increasing a shared deadline. It never resends the original prompts.

Validation

115 focused tests passed, plus vp run check:changed. Regressions cover multi-session recovery, repeated failures, deleted sessions with stale activity snapshots, Stop, background request ownership, process death, shutdown and subscription cancellation. The final independent review returned no findings.

Real Linux/OpenCode harness and before/after transcripts use OpenCode 2.1.0, Node 26.12.1, Linux arm64, a 1.5-core / 2-GiB container, and a local model fixture that drives real shell tools:

 • Same stall, original versus fixed: pause the owned OpenCode PID for 65 seconds, then delay seven real reconciliation responses by eight seconds each. The original adapter fails both T3 turns after five timeouts, although native history shows both executions succeeded and the same server remains alive. This branch completes both turns with the same PID and two initial model calls.
 • CPU contention: two turns, each running four CPU-bound worker threads, complete under the container quota without reconnecting.
 • Actual process death: killing the captured owned PID promptly fails both turns.

Loopback HTTP/SSE evidence additionally verifies Stop before a blocked history read is released and closure of both SSE sockets, including an unread recovery subscription. All provider state was temporary; no live user state or provider credentials were used.

Limit: the exact NixOS/systemd/zram starvation and upstream database-lock causes were not recreated. Controlled suspension and response delays reproduce the SSE silence, live process and slow recovery that T3 must tolerate. CPU scheduling and OpenCode's database behavior are unchanged.

Recovery policy

Important

A live owned process may remain in recovery until it answers, the user stops, it exits, or T3 shuts down. There is intentionally no elapsed-time cutoff that fails all its work. External-server recovery remains bounded.

Closes #23962

Model and harness: GPT-6.1 Sol, Codex in T3 Code.

📝 Walkthrough

OpenCode v2 recovery now distinguishes owned servers from external connections. Event requests follow their connection scope, and owned-process exit is available as a lifetime signal. Reconciliation and turn updates are coordinated with session state. Tests and provider documentation cover the updated recovery behavior.

Changes: OpenCode v2 recovery

Event-stream requests use a scope. Owned connections expose a process-exit signal, which the adapter races against stream following. Tests check request cancellation and process lifetime.
Reconnect and reconcile active turns
packages/provider-opencode/src/server/v2/adapter.ts
Owned servers retry reconciliation with capped exponential delays; external servers keep bounded retries and a timeout. Recovery mutations check the active turn under a lock. Reconciliation updates missing sessions and pending requests, and Stop handling finishes interrupted turns under the lock.
Recovery test controls and scenarios
apps/server/src/orchestration-v2/Adapters/OpenCode2AdapterV2.testkit.ts, apps/server/src/orchestration-v2/Adapters/OpenCode2AdapterV2.test.ts, docs/user/providers-opencode.md
Replay controls support delayed responses and simulated process exit. Tests cover stalled and repeated recovery, request restoration, cancellation, Stop behavior, and background work. The provider guide describes reconnection behavior.

Merge Risk: đŸ”” Low · up to 4aa8e

This change adds recovery tests for OpenCode and does not alter production behavior in the reviewed files. One test would hang rather than report a clear failure if the terminal status were wrong. This is a small test cleanup and does not block merging.

Explanation

The title "fix(opencode): preserve owned turns during server recovery" is a conventional commit title that matches the main change. The raw summary shows recovery changes for T3-owned OpenCode servers: reconnect with an exponential backoff capped at 30 seconds, reconciliation without a deadline for owned processes, and turns that remain active during recovery. The title is specific and does not include file lists or vague terms.

Full details: Description check

Explanation

The description follows the repository template. It has Behavior, Why, Validation, and Recovery policy sections, and it links issue #23962. The template's Problem, Change, and Scope and approval headings are not used as written. The Validation section lists focused tests, a changed-check command, and linked before/after harness evidence, and it names the environment and the limits of the verification. The description also gives the model and harness used. Its content covers the problem, the change, and the verification. The main gap is that it does not explicitly state the maintainer approval link or comment for scope, which the template asks for. The Behavior and Why sections give the problem and the change in substance.

Full details: Linked Issues check

Explanation

Directly linked issue [#23962] is open. Its requirement: T3 tolerates a stall of a live shared OpenCode server, or reconnects after the server answers, and does not fail every OpenCode thread after bounded retries.

Evidence at head 4aa8e263, verified in this review from the whole-PR diff against merge base 97be291 (adapter.ts and OpenCode2Server.ts):

 1. Owned-process recovery has no read deadline. In reconnect, connection.processExit === undefined selects the bounded path (reconcile.pipe(Effect.timeout(RECONCILE_TIMEOUT)), times: RECONNECT_ATTEMPTS - 1, Schedule.spaced(RECONNECT_DELAY)). Otherwise the path uses reconcile.pipe(Effect.retry({ schedule: OWNED_RECONNECT_SCHEDULE })) and times is undefined. The owned path therefore has no retry count or elapsed-time cutoff.
 2. Backoff is capped. OWNED_RECONNECT_SCHEDULE is Schedule.exponential("2 seconds") with Schedule.modifyDelay clamping each delay to 30 seconds.
 3. The signal that marks a server as owned is OpenCode2Server.withConnection, which now sets processExit: server.exitCode.pipe(Effect.asVoid). The owned-versus-external choice depends on this field.
 4. Reconciliation does not resend prompts. It reads history and pending requests, and it settles or updates turns that are already active.
 5. Turn ownership is rechecked under the lock. Backfill, request reconciliation, and turn completion each run inside lock.withPermit and check state.active !== turn or the request owner before they apply results. HTTP reads run outside the lock, so Stop can settle while a history read is blocked.
 6. A session that returns SessionNotFoundError is marked missing, removed from the active set, and finished as broken without blocking sibling sessions.

The issue's workaround (lowering tool priority) is not required. The issue lists longer recovery while the server stays alive as one valid direction, and the PR implements that direction.

Limits: the truncated part of the adapter.ts diff (after the reconnect hunk) was not visible. The summary states that recovery races the event stream against processExit, and the PR tests cover process death. I did not verify that race directly. The retry options are read from the initial connection when reconnect is defined. This assumes one adapter keeps one server kind for its lifetime, as the previous review noted. CPU scheduling is unchanged, as the PR states.

Full details: Out of Scope Changes check

Explanation

All changed areas connect to the recovery requirement of [#23962] or to the behavior named in the current PR description:

 • adapter.ts: owned and external recovery policies (verified in the diff), history backfill under the lock, missing-session handling for deleted sessions, request reconciliation that avoids re-asking for requests already pending, cancellation of requests absent from the latest listing, and unsettled-turn handling when interrupt times out during recovery. These implement recovery and Stop behavior described in the PR.
 • OpenCode2Server.ts: optional processExit on OpenCode2Connection, set from the owned process exitCode (verified in the diff). This is the process-death signal that recovery uses.
 • OpenCode2Client.ts and its test: events requires Scope.Scope, and /api/event runs under HttpClient.withScope. This closes unread SSE subscriptions with their connection, as the PR states.
 • OpenCode2Server.test.ts: checks that the spawned connection has an owned lifetime and that the process exits when the test scope closes.
 • OpenCode2AdapterV2.testkit.ts: beforeResponse and processExit options. These support the replay tests.
 • OpenCode2AdapterV2.test.ts: regression tests for recovery, Stop, background requests, deleted sessions, process exit, and shutdown.
 • docs/user/providers-opencode.md: documents the new recovery and Stop behavior.

The last commit adds controlled tests for requests that arrive during reconciliation, which also fits the recovery requirement. I found no demonstrated unrelated product change. Limit: the remaining part of the adapter.ts diff and the test-file diffs were not read line by line. Their scope rests on the file summaries and the PR description.

37## Summary
38
39Sessions whose rows only exist in the V1 `session` table are invisible in V2: they
40are missing from the session/tab list, `GET /api/session/<id>` returns 404,
41`opencode session export <id>` answers `Session not found`, and the desktop app
42shows "This tab points to a session that no longer exists on this server". The
43session data itself (messages and parts) is intact in the V1 tables — it is a
44read-path/projection problem, not data loss.
45
46## Environment
47
48 * opencode version: 2.0.28 (desktop app + its bundled CLI `cli/2.0.28-arm64/opencode-cli serve --service`); homebrew CLI on the same machine is 2.0.25
49 * OS: Darwin 25.6.0 (darwin arm64), macOS desktop app
50 * Terminal: Unavailable: desktop app (no `$TERM_PROGRAM`); reproduced with the CLI too
51 * Shell: /bin/zsh
52 * Install/channel: release/latest (desktop app 2.0.28 + homebrew 2.0.25)
53
54## Reproduction
55
56 1. On a profile that has been writing sessions with an older version (rows present in
57the V1 `session`, `message`, `part` tables), let some sessions be written only to
58the V1 tables (see Additional Context for the observed boundary).
59 2. Start opencode 2.0.28 against that same `opencode.db`.
60 3. Observe `GET /api/session/<id>` → `404` for those ids (4 ids requested on app start,
61in the restored tabs), and `opencode session list` / the tab list omitting them.
62
63Verified in SQLite:
64
65 select count(*) from session; -- 89
66 select count(*) from session_v2; -- 81
67 select id, title, time_created from session
68 where id not in (select id from session_v2); -- 9 rows, all invisible
69 * Every API call for them returns 404 (session, message, inbox, export).
70 * `opencode session export <id>` prints `Session not found`, so the documented
71export/import route cannot be used to rescue them.
72 * The V1→V2 projection appears to have stopped partway: 80 sessions have rows in both
73tables, but a contiguous range of later sessions (a ~2-day window) has V1 rows only.
74
75## Additional Context
76
77 * Observed boundary on this machine: the last session projected into `session_v2` was
78created `2026-10-11 15:37`; the first session existing only in `session` was created
79`2026-10-11 20:06`. Everything created up to the migration boundary is dual-present.
80 * The orphan rows carry `version = 1.18.35`; the app was updated to 2.0.28 afterwards
81(the first 2.0.28 session was written into `session_v2` correctly). So this looks like
82a partial/failed projection for rows written in between.
83 * For a session present in both tables, the V1 tables turned out to be a superset of
84what the projection stored: 293 V1 messages vs 213 exported, 81 messages never reached

483 * Session list cursors are opaque branded values carrying continuation query and ordering state. Consumers pass them back unchanged and do not inspect storage anchors or encoded filter fields.
484 * A Session list continuation accepts only its opaque cursor. Scope, filters, ordering, and page size are fixed by the initial query and carried by that cursor.
485 * `sessions.messages(...)` returns a Page and uses the same cursor discipline as `sessions.list(...)`: the initial request supplies `sessionID`, ordering, and page size; continuation supplies `sessionID` plus only an opaque branded message cursor carrying ordering, page size, direction, and message anchor. Using a cursor with another Session is invalid.
486 * `sessions.message({ sessionID, messageID })` is a required resource lookup. An unknown Session fails with `SessionNotFoundError`; a known Session with an absent or differently owned message fails with `MessageNotFoundError` without disclosing cross-Session ownership. Absence is not represented as `undefined` across the public HTTP boundary.
487 * `sessions.switchAgent({ sessionID, agent })` is part of the common client alongside `sessions.switchModel(...)`. It affects subsequent Session activity and fails with `SessionNotFoundError` for an unknown Session.
488 * The Embedded OpenCode Layer delegates to the same scoped creation path; it does not define a second implementation.
489 * A PTY Environment adapter observes plugins in the request Location while passing the resolved PTY working directory to the hook; standalone servers use an empty adapter.
490 * A Mid-Conversation System Message lowers to the provider's native chronological instruction role when supported and to a wrapped chronological fallback otherwise.
491 * When the effective aggregate instruction set changes, its Mid-Conversation System Message includes the complete current ordered set and supersedes the prior aggregate value; when no ambient instructions remain, the message states that previously loaded instructions no longer apply.
492 * Ambient project instruction discovery honors `OPENCODE_DISABLE_PROJECT_CONFIG`; global instructions remain eligible.

530The issue is still present in the latest version (2.0.28) when using OpenCode inside a VM.
531
532It's becoming a major trade-off, especially since `/share`, which I previously relied on as a workaround, now returns:
533

535
536At this point, I can't even extract text from conversations without using OCR or switching to a free model to ask OpenCode to upload the content somewhere.

Handle deleted sessions during backfill.

When OpenCode returns SessionNotFoundError from client.message.list, backfill propagates the error. Owned-server reconciliation then retries indefinitely, so the new event stream is not followed and other sessions can stop receiving updates.

✅ Addressed in commit 119d899

Confirmed and fixed in 119d8995ca. The regression deletes one of two sessions after session.active has already listed both as running. Backfill now handles SessionNotFoundError locally, removes the deleted session from that stale activity snapshot, and ends only its turn as interrupted/broken. The surviving session reconciles and completes; the deleted session is also skipped when restoring permissions/forms. The regression failed before the fix and passes afterward. All 114 focused tests and check:changed pass.

@Zheruel The supplied code addresses this finding. backfill handles SessionNotFoundError locally, and reconciliation removes the deleted session from the activity snapshot before restoring permissions/forms. Only the deleted session’s turn ends as interrupted/broken, so sibling sessions can continue recovery.

Thanks for adding the regression test. I have not independently run the reported checks.

Preserve requests created after the recovery list starts.

permission.list and session.form.list can read their pending state before OpenCode creates a new request. OpenCode then inserts the request before publishing its event. The event can arrive while Effect.all is still waiting, so replay adds the request to pending after listed was built. The final sweep then calls settleRequest, which removes T3's entry without answering the native request.

 • test(opencode): cover requests arriving during reconciliation38f1cdc
Synced from GitHub

@coderabbitai The outside-diff finding in this review does not apply to the current reader ordering. reconnect awaits all of reconcile, including its final sweep, before returning the replacement stream. follow starts processing that stream only afterward. Events can arrive over HTTP during the list calls, but they cannot populate pending during that sweep.

Added a controlled case in 38f1cdcba1: both list snapshots are empty, the form response is held, and a new approval event arrives on the replacement stream. No request is processed while recovery is held; afterward both the approval and question remain answerable and the turn completes. Production code is unchanged. All 115 focused tests and check:changed pass.

📐 Maintainability & Code Quality | 🟡 Minor | ⚡ Quick win

Move the terminal-status check out of the forked fiber.

assert.equal(event.status, "completed") runs inside a fiber that Effect.forkScoped creates. The test scope does not join that fiber. If the status is wrong, the fiber fails and Deferred.succeed(ended, undefined) never runs. yield* Deferred.await(ended) at Line 2507 then waits until the project timeout. The test does not show the actual cause. Pass the status through the Deferred and check it after the await.

thanks @Zheruel. your PR correctly ties owned-server recovery to process lifetime, keeps Stop out of blocked history reads, and preserves turn/request ownership and SSE cleanup.

rebuilt on current main as #25596 because this fork has maintainerCanModify: false. it retains your recovery policy, missing-session isolation, stop handling, request reconciliation and regression cases, with your release credit in the commit trailer. this PR stays open.

i compared your full diff with the replacement. no recovery mechanisms or regression cases were dropped or moved. current main's surrounding changes were retained, the history comment was shortened, and the terminal assertion now runs in the test's main fiber instead of potentially stranding its Deferred.

120 focused tests and Blacksmith vp run check:changed pass. real OpenCode 2.1.0 reproduces false failures on main and completes both turns after the same stall on the replacement, with no resubmission. real process death also fails both turns. the replacement body links the transcripts and records unverified client/platform paths. both independent code reviews approve; CI and automatic reviews are pending.
