Fixes #23600

Problem

Stopping a thread while it shows "Preparing worktree" interrupts the run before any worktree exists. The next message has no active run, so it takes the start_immediately path. The guard that holds messages after a failed or stopped worktree setup (worktreeMissingRun) only applied to queue_after_active, so the message skipped it and ran in the project's main checkout, with no warning. The reporter's local checkout was edited by the agent.

Fix

worktreeMissingRun now also applies to start_immediately. After a failed or interrupted worktree setup that left the thread without a worktree, a message sent fresh waits, held, like one queued during setup. Retrying the preparation releases it, as before.

This changes one asserted behavior. The test "holds a message queued during setup that arrives after the setup %s" previously expected a fresh auto message to start in the project checkout while the earlier message stayed held. It now expects the fresh messages to wait as well (renamed, with the run states and worktreePath asserted).

Evidence

  • The updated test fails on main for both the failed and interrupted setups and passes with the change.
  • vp run check:changed: format, lint, knip, typecheck and the 3 affected test files (50 tests) pass.

Not verified / out of scope

  • I did not run the desktop app. The reproduction is the unit-level equivalent of the issue's event sequence.
  • The issue's secondary item (the cancelled preparation's prepared-run.fail dispatch being rejected with "not awaiting workspace preparation") is a harmless failure span and is not changed here.
  • Whether the resend should instead re-prepare the worktree automatically is a product call; this keeps the existing hold-until-retry behavior.

Approvability

Verdict: Approved at eec3b71

Macroscope's review found this PR approvable — This small server fix ensures messages sent after an unsuccessful or interrupted worktree setup remain held until a worktree is available, rather than executing in the project checkout. The behavior is directly covered for both failure modes and repeated follow-up messages.

Walkthrough

The orchestrator now holds immediate messages after failed or interrupted worktree preparation when the thread has no worktree. The updated test checks that subsequent messages remain queued and held.

Changes: Worktree setup message handling

The missing-worktree check now includes start_immediately messages. The test verifies that multiple messages sent after setup ends remain queued and held, and that the thread has no worktree path.

Messages sent after a failed or interrupted worktree setup are now held, which fixes the reported silent run in the main checkout. However, pressing Resume or Start on a held message can still run it in the project checkout without a worktree. Decide whether that is intended, or make promotion retry or require worktree preparation, before merging.

Explanation

The PR satisfies the coding requirement in issue #23600. Orchestrator.ts applies worktreeMissingRun to both queue_after_active and start_immediately when the latest run failed or was interrupted during worktree preparation and the thread has no worktree. The message remains held instead of starting in the project checkout. ThreadLaunchService.test.ts covers failed and interrupted setup and asserts queued, held runs with worktreePath unset. The unchanged prepared-run.fail behavior is secondary and is not required for this fix.

updated the original branch at 3f8e1cb7cd4c323df35eb5bb5f8b2e8c86f59109, preserving @Gourav224's commits and merging current main. the original guard and regression coverage were already focused; i kept them and shortened the guard comment. the final diff is two files, 19 insertions and 11 deletions.

verification:

  • both regression cases fail against main (0a3b561bc090): fresh messages start instead of queueing after failed or interrupted setup.
  • blacksmith: all 41 launch tests pass. vp run check:changed passes format, lint, knip, graphql, affected typechecks and three affected test files.
  • real chrome/web, sqlite, git and codex: main starts a fresh message in the project checkout after setup is stopped. restoring the identical stopped thread on this head holds that message with no provider session. failed setup also holds a fresh message.
  • clicking retry after failed setup creates the worktree and completes both the original and held runs. codex's pwd response and its provider session cwd both match the new worktree.
  • github ci build, typecheck, lint and all eight server test shards pass. two independent reviews found no issues and cleared the runtime verification. coderabbit is green. its explicit-promotion finding was checked against main, documented as an existing limitation, replied to with @Gourav224 tagged, and resolved.

unverified: electron, native mobile, other providers, windows/macos and relay/tunnel connections. this changes the shared server dispatch path without changing client or provider contracts. explicit manual queue release remains existing behavior and is outside this fix.

before, on main with the same stopped thread and prompt:

main starts a fresh message after worktree setup was stopped

after, on this head:

the same fresh message stays queued after stopped worktree setup

another fresh send remains held too:

a repeated fresh message joins the held queue

Verified in a real dev environment

verified as-is at 3f8e1cb7cd4c323df35eb5bb5f8b2e8c86f59109 on this linux boat vm. no source changes or pushes.

  • installed and ran github/main (eb751fa073) in /home/user/main, then stopped its captured process group before installing and running the exact contributor head in /home/user/t3code. only one dev server ran at a time, with isolated state.
  • used the repo playwright dependency and /usr/bin/google-chrome. created the project and threads through the web app, pointing at a real git repository. used codex gpt-6.1-sol through the local proxy. no mocks, fixtures or replay provider.
  • reproduced main: selected new worktree with start from remote, paused a real local git daemon during fetch, and clicked stop before checkout. a fresh message then completed in /home/user/verification-23618 with worktreePath: null.
  • repeated that flow on this head with the same repository and prompts. two fresh sends stayed queued and held, with no worktree and zero provider sessions for that thread.
  • separately failed the real remote fetch. after reloading the client, a fresh send stayed held. restored the remote and clicked retry. both the original message and held follow-up completed, reporting the new worktree directory. the worktree path and codex session cwd matched /home/user/t3code/.t3/worktrees/verification-23618/t3-c5c5da7e.
  • a new current-checkout thread still completed its pwd turn in /home/user/verification-23618.
  • blacksmith: vp test run apps/server/src/orchestration-v2/ThreadLaunchService.test.ts, all 41 tests passed. the existing head also has 29 successful ci contexts, no failing or pending checks.

full focused runtime transcript

before on current main, fresh send runs in the project checkout:

main runs the fresh message in the project checkout after cancelled worktree setup

after on the exact pr head, the same fresh send stays held:

pr holds the fresh message after cancelled worktree setup

failed setup also holds a fresh follow-up

retry completes the original and held turns in the new worktree

stop worktree setup and send two fresh messages that remain queued

Not verified

  • electron, native ios/android, macos/windows, other providers, and remote/relay/tunnel connections were not exercised on this linux vm.
  • after the immediate fetch failure, this client left "preparing worktree" visible and disabled fresh submit until reload. reload cleared it; the server hold and retry behavior then passed. the cause and whether current main also exhibits that ui issue were not investigated. source was left unchanged.
  • explicit resume/send-now promotion without a worktree was not exercised here. the discussion already records it as an existing limitation outside this automatic-dispatch fix. interrupted setup has no visible retry action in this head; recovery was verified through failed setup retry.
