Problem

A nested Claude subagent can remain running after it completes. Reproduce by letting a background parent launch a child while the root is idle, completing that child during a user turn, then letting a sibling completion wake the root: the buffered start replays without its completion.

Change

Keep a live completion notification behind its matching buffered start when the task has not been registered yet. The continuation replays both in order and persists the child as completed. Registered-task handling remains unchanged.

Scope and approval

Fixes #16355

Triaged bug via the automated issue flow. This fixes the reproduced completion-ordering defect in the Claude adapter with one regression test.

Verification

  • Regression fixture fails at the test-only commit with expected 'running' to equal 'completed', then passes with the fix.
  • cd apps/server && vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts: 208 passed on the final head.
  • Server npx tsc --noEmit: passed. Targeted formatting passed; adapter lint reports the existing unused layer warning, also present on base.
  • Two isolated code reviewers returned MERGE with no confirmed findings. Final proof audit returned SUFFICIENT.
  • Real web development server on Linux, Claude Opus 5.5 / CLI 2.1.303: repeated the idle-child / user-turn completion / sibling-wake sequence. Before, the child persisted as running and the UI waited on it. After, it persisted as completed with a completion timestamp and zero running subagents.

Server-only proof: fixture matrix has zero changed render paths. Evidence index includes the ordered runtime events and recording timestamps.

After lifecycle recording: prompt at 00:01, user follow-up at 02:06, continuation complete at 03:47. Screenshots and recording supplement the server proof; message history, timestamps, and restart notices differ, so these are not pixel comparisons. Chromium 154, 1280×800 CSS pixels, DPR 2, light theme, default font scaling, no device emulation. No client rendering code changed.

Implemented by Claude Opus 5.5 through the Claude Code harness; coordinated and reviewed by GPT-6.1-sol through Codex, with an independent Claude Opus 5.5 review.

Approvability

Verdict: Approved at 699cc4b

Macroscope's review found this PR approvable — The adapter preserves completion notifications alongside buffered task starts so nested subagents can retain their completed state across turns. The change is limited to existing session-event replay, with matching-task and duplicate-message guards.

No code changes detected at 9e22c87. Prior analysis still applies.

Walkthrough

The Claude adapter now buffers an early task notification behind a matching buffered start when the task is not registered. A regression test checks that a nested subagent remains completed after a sibling-triggered continuation settles.

Changes: Claude subagent completion

Layer / File(s)Summary
Buffer early notifications and verify completion
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The adapter buffers an early notification when the wake buffer contains the matching task start. The regression test checks the nested subagent’s final status and confirms no background work remains after the continuation settles.

Explanation

The description includes the required Problem, Change, Scope and approval, and Verification sections. It explains the defect, implementation, linked issue, focused tests, observed results, and limitations.

Full details: Linked Issues check

Explanation

[#16355] Only one directly linked issue applies. Its coding requirements are: (1) apply a buffered completion when the matching start is not yet registered, so the child does not stay running; (2) reach a terminal status with a completion timestamp; (3) keep idle-buffered starts and completions applied at the next drain; (4) complete a held start when its completion arrives while idle; (5) keep genuinely running subagents running. The whole-PR change summary states that, when a task is unregistered and its task_started frame is already buffered for the same native thread, ClaudeAdapterV2.ts appends the completion notification to the wake buffer (unless already present). Registered tasks stay on the existing outcome path, and unregistered tasks without a buffered start are not added. This matches requirements (1), (2), and (5). The regression test in ClaudeAdapterV2.test.ts launches background subagents, runs an idle nested child to completion, then processes the sibling completion and wake turn, and asserts the nested child ends completed with no background work left. The reported run of 208 tests passed on the final head, and the PR reports a failing fixture before the fix (expected 'running' to equal 'completed'). Limits: the source diff could not be read because changed blobs are missing from the checkout, so the verdict relies on the whole-PR change summary, the test description, and the reported results. The adapter file could not be inspected for the specific idle-completion and held-start branches (requirements 3 and 4), and the changes since the previous reviewed head (92f1042d8c) were not verified in source. The described behavior and the existing test suite support compliance.

Full details: Out of Scope Changes check

Explanation

The whole-PR change summary lists two changed files: apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts (+26/-4) and its regression test ClaudeAdapterV2.test.ts (+192/-0). The adapter change addresses the buffered-start and completion ordering defect in [#16355]. The test covers the same lifecycle path. The summary reports no changed declarations of exported or public entities. The PR description reports no client rendering change, and its supporting proof material is external to the repository. The screenshot in the linked PR objectives shows +215/-4, while the summary counts +218/-4. This gap is small and consistent with a snapshot taken before a later commit, so it does not show unrelated work. Source diff access was unavailable, so the file list relies on the change summary rather than a direct diff read.

Count buffered Claude lifecycle messages as pending work.

When an unregistered task_notification is appended behind task_started, wakeBuffers can contain only those two messages while a continuation is pending. Both Claude pending-work probes return false for this state. ProviderSessionManager can then release the idle session or unload the idle thread. The continuation drain can fail because the query is stopping, before it applies the buffered completion.
