fix(server): auto-resume retries once after a refusal at the usage-limit reset#22063

Problem

Auto-resume sends "Continue where you left off." as soon as the reported reset time passes. Claude can refuse that turn and report the same reset time. That time is now earlier than the new run's completion, so the worker guard and the candidate query skip the thread. It stays failed with nothing scheduled.

Change

  • Auto-resume waits 120 seconds past the later of the reset time and the failed run's completion.
  • If Claude refuses the resumed turn with the same reset time, recovery re-arms once and resumes 120 seconds after the refusal. A second same-reset refusal leaves the thread failed as before, so a past reset still cannot start a retry loop. The cap comes from persisted runs and recovery state, so it holds across restarts. A manual continuation refused with the same reset counts as that one retry.
  • Stop cancels the retry like any pending continuation (#21114). Turning auto-resume off for the thread cancels the retry. Turning it off in settings before the re-arm prevents it.
  • A continuation delivered before it is due starts nothing and leaves the worker's later delivery intact.

The 120-second margin comes from the reporter's logs, where resuming 120 s after the reset never drew a second refusal. Claude does not guarantee it. The candidate SQL repeats the TypeScript rules because it cannot call them. Existing completion-time fallbacks are unchanged. No UI code changed.

Scope and approval

Closes #22026. Maintainer triage accepts the bug: a single refusal at the boundary should not end auto-resume.

Verification

The tests run the real Orchestrator, event sink and SQLite projection, with Claude's same-reset refusal injected as provider events. They do not hit a live Claude limit or run the wall-clock scheduler. They cover the margin, the single re-arm and its cap (a manual re-arm request is rejected, also after Stop, and a refused manual continuation leaves nothing scheduled), per-thread cancel, the global setting off before the re-arm, Stop before and after the re-arm, and an early delivery followed by exactly one due continuation. They also check that a fresh SQLite read and an event replay return the same candidate.

  • 136 tests pass in apps/server/src/orchestration-v2/Orchestrator.control-reads.test.ts, apps/server/src/orchestration-v2/Orchestrator.migration.test.ts, apps/server/src/orchestration-v2/ProjectionStore.test.ts, apps/server/src/orchestration-v2/runtimeLayer.test.ts.
  • With apps/server/src/orchestration-v2/UsageLimitRecoveryWorker.ts, apps/server/src/orchestration-v2/ProjectionStore.ts, apps/server/src/orchestration-v2/Orchestrator.ts reverted to 066c8fc0cd, 6 tests fail with assertion errors: "usage-limit recovery > guards a scheduled usage-limit continuation against reset-margin", "usage-limit recovery > guards a scheduled usage-limit continuation against refused-boundary", "usage-limit recovery > guards a scheduled usage-limit continuation against refused-boundary-stop-reenable", "usage-limit recovery > guards a scheduled usage-limit continuation against refused-boundary-cancel", "usage-limit recovery > guards a scheduled usage-limit continuation against refused-boundary-off", "usage-limit recovery > guards a scheduled usage-limit continuation against refused-boundary-stop-armed".
  • vp fmt --check and vp lint report no errors and no new warnings on the 6 changed TS files.
  • Typecheck passes for t3.

Walkthrough

Usage-limit auto-resume now applies a 120-second margin after the later of the reset time and run completion or request time. Candidate selection and continuation checks share recovery timing and stale-reset rules. Runtime tests cover retry boundaries, rearming, cancellation, and stopping.

Explanation

The description covers the problem, implementation, scope approval, cancellation behavior, limitations, focused verification, test results, tooling checks, and agent attribution. It follows the required template and directly matches the reported changes.

Full details: Linked Issues check

Explanation

Issue [#22026] requires delayed usage-limit auto-resume and a later retry after a same-reset refusal. UsageLimitRecovery.dueMs applies a 120-second margin after the later of resetAt and run completion. The worker, projection candidate query, and Orchestrator use this timing. staleResetAllowed permits one persisted stale-reset re-arm and rejects a second one. The changes also cover manual continuation, Stop, auto-resume setting changes, cancellation, early delivery, and replay consistency. The added orchestration tests cover these requirements.

Full details: Out of Scope Changes check

Explanation

The changes stay within the server usage-limit recovery path for issue [#22026]. UsageLimitRecovery, UsageLimitRecoveryWorker, ProjectionStore, and Orchestrator implement the recovery behavior. The test changes verify timing, retry limits, cancellation, setting changes, Stop, and projection replay. The test-layer change supports these tests. No unrelated UI or feature changes are shown.

Consume the automatic recovery arm when accepting a manual usage-limit continuation.

When the manual continuation is accepted, the existing limitRecovery arm remains enabled. If that manual run is refused after the same reset, the new SQL branch admits it because the reset is later than the earlier armed run. The in-memory selector permits the same state. UsageLimitRecoveryWorker then creates a new automatic arm and retries again, exceeding the single-manual-retry cap.

Clear the matching arm before starting the manual run. This blocks the introduced post-reset path in both selectors without changing the automatic-first-refusal path.

✅ Confirmed as addressed by @ScottN-PV

Reproduced on 0a7d18479f: after an automatic retry was refused, a manual continuation refused with the same reset left the original arm enabled, so the candidate query returned the thread and the worker's command path started another run. 23cbc0743f clears the arm when a manual usage-limit continuation is accepted and the arm has the same reset, and the new refused-boundary-manual case checks that the store, a fresh SQLite read and an event replay return no candidate. It matches resetAt rather than runId because in this sequence the arm still names the original run, not the retry being continued.

Written by Claude Opus 5.5 on behalf of ScottN-PV.

@ScottN-PV Thanks for the fix and clarification. Matching resetAt is correct here: the automatic recovery arm can still reference the original run, not the retry being continued. My suggested runId check was too restrictive.

Clearing the same-reset arm addresses this finding. The reported refused-boundary-manual coverage checks candidate exclusion across the store, a fresh SQLite read, and event replay.

5Learning: In apps/server/src/orchestration-v2/Orchestrator.ts, an automatic usage-limit recovery arm can retain the original runId after an automatic retry is refused. When accepting a manual usage-limit continuation, match the enabled recovery arm by resetAt rather than requiring its runId to match manualContinuationOfRunId. Clear that same-reset arm so a refused manual continuation cannot trigger another automatic retry.

Prevent Stop from re-enabling a cap-exhausted recovery.

After the second same-reset refusal, a direct thread.metadata.update with autoResume: true is rejected. However, thread.stop rewrites the recovery with the latest failed run ID and autoResume: false. A later client metadata update with autoResume: true then passes the same-run stale-reset check. Once due, limitRecoveryCommand schedules another continuation past the retry cap.

Allow same-run stale records to cancel or patch recovery, but reject a same-run stale record when the update explicitly enables auto-resume.

  • fix(server): retry usage-limit refusals at the reset boundary6cde0e5
Synced from GitHub

Fixed in 6cde0e580f: the outside-diff finding reproduced through a direct thread.metadata.update after Stop, and the current banner and mobile card hide that action. The suggested autoResume === true gate alone still accepted the update, because staleResetAllowed passed a stopped same-run record, so the helper now also requires the stored arm to be enabled. The new refused-boundary-stop-reenable case covers Stop after the cap, then cancel and snooze patches (accepted) and re-enable (rejected).

thanks @ScottN-PV. your persisted retry bound fixes the same-reset stall without introducing a retry loop. i kept your implementation and commit, merged main, and added a small follow-up.

  • web and mobile keep cancel available during the retry wait, including before the scheduler updates the arm's run id. enabling recovery after the cap remains blocked.
  • the refusal tests now re-arm through the scheduler's autoArmLimitRecovery path. an added case verifies cancellation wins over a stale re-arm. i removed the unrelated empty-thread stop assertion. no retry timing or cap changes.

verification: six regression cases fail against current main's server code. testbox vp run check:changed passes format, lint, both knip checks, graphql validation, four package typechecks, and 161 tests in six files. two independent final reviews approve a3beeb61a6b2713a5198ae8a239eeb4a2e393139, including scheduler review. web cancellation ran through the real client/server command path and persisted autoResume: false.

ci: all 28 checks pass on this head, with no pending or failing checks.

unverified: live Claude quota exhaustion, native mobile, Electron, Windows/macOS, and relay/tunnel routes. the screenshots use synthetic post-refusal projection data; they prove cancellation presentation and delivery, while the runtime tests cover recovery timing, cap, replay and cancellation races. the recording is cropped to the composer.

  • ran vp test run apps/server/src/orchestration-v2/runtimeLayer.test.ts -t "usage-limit recovery" through blacksmith-check: 26 passed, 72 excluded by the name filter. these are automated regression checks with injected provider events, not live quota proof. the initial testbox hydration timed out; the same worker succeeded on retry.

the animated recording shows neighboring stop/resume behavior, with an idle pause removed. it does not show usage-limit recovery. the screenshots show the same limits panel on main and the pr. no mocks, seeded refusal events, fixtures, or source changes were used for the browser scenarios. both dev servers and the testbox have been stopped.

main: the real codex proxy cannot report usage limits

pr head: the same real codex usage-probe failure

pr head: stop a real codex tool run and manually resume it, idle pause trimmed

Not verified

the original bug could not be reproduced on main. the configured codex cli proxy completes real turns but its usage probe returns Codex could not read usage (JSON-RPC -32600) on both revisions. no real turn returned a usage-limit refusal or reset timestamp. consequently, i could not exercise the two-minute delay, same-reset re-arm, retry cap, recovery cancellation, or recovery across a restart through the live provider path. i did not substitute synthetic failures or exhaust the account to manufacture a limit.

this needs a real quota-limited provider account that reports a reset time and refuses once after it. native ios/android, electron, macos/windows, and relay/tunnel routes were not exercised on this linux vm. github reports no failing or pending checks on this head, but that does not close the live recovery gap. no commits were made or pushed.

merged github/main (97e35bcf23e4) without rebasing. kept main's removal of the in-memory projection store and moved the existing recovery replay assertions to isolated sqlite. two independent reviewers caught layer memoization sharing the runtime database; the follow-up uses fresh layers with scopes covering replay/query, verifies replay starts empty, and checks the runtime projection stays unchanged. both reviewers approve the final head with no findings.

blacksmith vp run check:changed 97e35bcf23e4dcf34c2dcffadd37c08db0e01e52 passes format, lint, both knip checks, graphql, five package typechecks, 175 tests across seven files, and six compiler checks. the focused usage-limit recovery run also passes all 26 cases. github reports CLEAN / MERGEABLE, with no pending or failing checks; git.cafe reports mergeable and 29 successful contexts (including github's skipped jobs). the existing bot findings are addressed and the inline thread is resolved.

scope remains eight files: 381 additions and 29 deletions against main, compared with the starting 385 additions and 32 deletions. no production behavior or UI changes were added in this pass. pushed to the existing contributor branch without force. no merge or label changes performed by this agent.

unverified: live provider quota refusal/reset recovery, native mobile, electron, and relay/tunnel paths. prior evidence and its limits remain in the discussion; this pass adds no new browser evidence. no dev server was started, and the blacksmith worker is stopped.
