Turn starts in long threads now block the server for 30–46 ms instead of 314–369 ms, measured with the real recovery read on a copy of a 15.8 GB database.

Problem

Starting a turn in a thread with about 14,000 or more turn items blocks the server's main thread for 363–445 ms in live traces (314–368 ms measured on a copy of the database). While it is blocked, no client gets an answer, so typing and clicks in the browser preview and every other connected client wait.

The time goes to four calls of ProjectionStore.getRuntimeRecoveryProjection per turn start. Its queries look up the thread's unfinished turn items without a filter on type, and every existing partial index on turn items filters on type, so each lookup reads the thread's whole history. Since #21122 the server runs without planner statistics and relies on indexes, so nothing else narrows it.

Change

  • Migration 068 adds a partial index on orchestration_v2_projection_turn_items(thread_id, ordinal, turn_item_id) holding only unfinished items (status IN ('pending', 'running', 'waiting')). The key order also serves the query that orders by ordinal.
  • The runs query in getRuntimeRecoveryProjection drops AND item.run_id IS NOT NULL from its subquery. It filtered nothing (a NULL never matches IN), but it led SQLite to pick the run-id index over the new one.
  • A test in ProjectionRecovery.test.ts checks the query plan of every turn-item lookup in the recovery read uses one of the partial indexes. It fails with the index removed.
  • Migration list tests include 068.

Scope and approval

Fixes #21578, triaged as bug and accepted. The issue has the measurements from the live server.

Related: #20302 proposed the same index and was closed without a measurement through the app; this one is measured on the server's own recovery read, with the stalls traced on a running server.

Verification

How the problem was found: on my live server (v0.1.1-nightly.20261018.3335, WSL, 15.8 GB database), a request every 50 ms timed the event loop. Every stall over 300 ms lined up with a turn start, and the server trace spans inside each stall (363–445 ms) were these four recovery reads.

How the fix was measured: the real ProjectionStore.getRuntimeRecoveryProjection, called four times as one turn start does, on a copy of that database with planner statistics cleared as the server does at startup. Warm cache, median of 5 rounds, longest event-loop gap from perf_hooks.monitorEventLoopDelay. Before is 1451e7cb23 without the index; after is this branch.

Thread turn itemsBefore: wall / longest gap (ms)After: wall / longest gap (ms)Change
17,517367.5 / 369.129.6–37.7 / 31.0–39.1~10–12x shorter
13,885313.9 / 315.429.9–36.8 / 31.2–38.1~8–11x shorter
13,699330.0 / 331.440.3–45.1 / 41.6–46.0~7–8x shorter

After ranges cover two runs. Building the index on the 15.8 GB database took 0.4–0.9 s warm and 7.3 s cold; the index is about 340 KB.

Tests: vp test run on apps/server/src/orchestration-v2/ProjectionRecovery.test.ts, ProjectionStore tests and the migration tests under apps/server/src/persistence/ pass. Server typecheck and lint pass.

At 759566fba6, all 23 tests across ProjectionRecovery.test.ts, migrations 055, 064 and 066, and reconcileV2PreviewMigration.test.ts pass on Node 24.22.0. Server typecheck, targeted lint and formatting pass.

Not checked: the extra write cost of the index (about two index writes over an item's life) was not measured, and I did not measure a turn start end to end on the patched live server.

Approvability

Verdict: Approved at acb154c

Macroscope's review found this PR approvable — This narrowly scoped server fix removes an ineffective query predicate and adds a partial index so recovery reads inspect only unfinished turn items, addressing the measured long-thread stall. The migration and query-plan tests cover the production-relevant changes without introducing new capabilities or product-default changes.

📝 Walkthrough

The runtime recovery query no longer requires matching unfinished turn items to have a non-null run_id. Migration 69 adds and registers a partial index for active turn items. Tests check migration expectations and index use in recovery query plans.

Explanation

The PR meets the coding requirements in directly linked issue [#21578]. Migration 069 adds a partial index on (thread_id, ordinal, turn_item_id) for unfinished items with status pending, running, or waiting. ProjectionStore.getRuntimeRecoveryProjection removes the redundant run_id IS NOT NULL condition. ProjectionRecovery.test.ts captures seven item lookups and requires every lookup to use turn_items_active_idx or turn_items_recovery_idx. The reported measurements show the four recovery reads reduce the long-thread event-loop gap to about 31–46 ms.

Explanation

The changes stay within [#21578]. The migration, migration registration and assertions, recovery-query adjustment, tracer-based query-plan regression test, and test instrumentation all support the long-thread recovery-read fix. No unrelated product behavior or unrelated code change is shown.

Bind the plan assertion to the final turn-item query.

The test aggregates plans from every traced statement. It also accepts turn_items_recovery_idx. A plan from another lookup, or the older recovery index, can therefore satisfy the assertion without proving that turnItemRows uses turn_items_active_idx.

A long thread with many finished user_message or assistant_message items can still regress to reading its history if the final lookup loses the active index. Filter the traced statements to that lookup and require the active index.

Checked this against 759566f. The assertion runs for every captured item lookup, so another query's good plan cannot satisfy it for a bad lookup. I temporarily added NOT INDEXED only to the final turn-item query; the existing test failed on SCAN item, even with the other lookups unchanged. Separately, dropping the active index also made it fail. The unchanged test passes with the index present. The older recovery index filters by item type and cannot serve the final lookup, which has no such filter. Keeping the broader assertion preserves coverage of all recovery item lookups. No code change needed.

Assert that all seven item lookup plans are captured.

assert.isNotEmpty(itemLookups) requires only one plan. A future edit could remove one lookup while the remaining plans still satisfy this test. Assert the expected count and keep the per-lookup index checks. Turn-item fixtures are not required for this query-plan check.

  • test(server): require all recovery item lookup plans79adb8e
Synced from GitHub

Addressed the count assertion finding from review 5596227786. The test now requires exactly seven captured item lookup plans and retains every per-plan partial-index check.

Verified the coverage gap with a temporary production-query mutation: renaming the final item alias and adding NOT INDEXED made its full scan escape capture. The old assertion passed with six captured plans; the new assertion failed with expected 6 to equal 7. Both mutations were restored.

All 13 ProjectionRecovery tests, server typecheck, and targeted lint/format checks pass.

Synced from GitHub

Closing in favor of #23557, which adds a partial index on unfinished turn items for #21578. This PR did the same, with two differences: its index key also holds ordinal and turn_item_id, and it drops AND item.run_id IS NOT NULL from the runs subquery, because SQLite picked the run-id index while that predicate was there. I have not measured whether either difference matters on top of #23557.
