Problem

An asynchronous delegated child can pause for approval after its parent finishes without notifying the parent. Reproduced with the real web dev server on Linux and Claude Opus 5.5: the child showed a pending command approval while the parent remained completed.

Change

Extend the existing delegated-question reactor to notify parents about live child approval requests. Notices link to the child thread and tell the parent to ask the user to handle the approval there; approval authority stays unchanged.

Reuse existing request IDs for deduplication. Withdraw queued notices when requests end or tasks stop, reject stale dispatch after disposal, and preserve blocking-wait delivery without announcing the same returned request twice. Update the existing delegation guide to match blocking-wait behavior. Contributor and maintainer history from this PR is preserved.

Scope and approval

Fixes #22925 GitCafe: https://git.cafe/pingdotgg/t3code/issues/22925 Triaged bug via the automated issue flow; issue #22925.

Verification

Behavior checked at: 152bad3669.

  • Baseline regression failed with expected +0 to equal 1 for the missing parent notice. Cancellation/expiry regressions also failed before their fix (queued instead of cancelled).
  • Orchestrator.control-reads.test.ts: 18 passed at the final head, covering approval notices, questions, live-only routing, withdrawal, deduplication and blocking-wait catch-up.
  • DelegatedCompletionDelivery.test.ts and ThreadStop.test.ts: 65 passed at the final head.
  • Server package typecheck passed at the behavior-checked head. Targeted lint passed with no errors; the pre-existing unused layerUnavailable warning remains. git diff --check passed.
EvidenceResult
Before: real-app reproductionParent completed before the child approval appeared; no parent wake followed. The full-access control did not prompt.
After: final-head runtime transcriptTen scenarios passed using the real event subscriber, isolated SQLite projections and orchestrator. Provider requests and parent completion events were synthetic; effect worker disabled. Covers all five approval kinds, guidance/link, internal exclusions, resolution/disposal, catch-up, returned-wait deduplication, cancellation and expiry.

The subsequent 8aadcc59a9 adds only an explanation of the existing tracing boundary; targeted formatting and diff checks passed. Runtime code is unchanged.

The visual fixture matrix is empty: no client render paths change. No real-provider after-fix acceptance is claimed; that attempt failed before delegation. macOS Electron, iOS, Android, and remote/tunnel acceptance remain unverified. The runtime transcript retains a trailing warning from the isolated fixture closing SQLite after assertions; the proof audit independently reproduced cancellation and identified this teardown race. No clean-shutdown claim is made.

Models/harness: GPT-6.1 Sol through Codex and Claude Opus 5.5 through Claude Agent, orchestrated in T3 Code.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval β€” This PR changes production orchestration behavior by automatically notifying parent threads about delegated-child approvals and modifying shared queue cancellation and stale-dispatch handling. The cross-cutting, user-visible runtime impact warrants human review.

πŸ“ Walkthrough

The delegated-task reactor now routes eligible questions and approvals to parent threads. The orchestrator validates notices before dispatch and cancels queued notices in specified disposal cases.

Changes: Delegated request notices

Layer / File(s)Summary
Request notice routing
apps/server/src/orchestration-v2/DelegatedTaskRequestReactor.ts, apps/server/src/server.ts, apps/server/src/orchestration-v2/Orchestrator.control-reads.test.ts
The renamed reactor distinguishes approval notices, which direct users to the child thread, from question notices, which retain read-and-respond guidance. The server starts the reactor. Tests cover approval notice content.
Request catch-up and parent run timing
apps/server/src/orchestration-v2/DelegatedTaskRequestReactor.ts, apps/server/src/orchestration-v2/Orchestrator.control-reads.test.ts, docs/orchestration-v2/orchestrator-mcp-server.md
Catch-up skips terminal tasks and handles pending requests according to task settings and response capability. Tests and documentation cover deferred notices and blocking-wait behavior.
Queued notice cancellation and dispatch
apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/orchestration-v2/Orchestrator.control-reads.test.ts
The orchestrator cancels queued notices when a task or its completion cohort is disposed. It rejects notices that reference tasks with invalid ownership, thread, or lifecycle state. Tests cover cancellation and late dispatch.

Possibly related issues

  • pingdotgg/t3code#22925: The change adds parent notifications for delegated child approval requests and handles their lifecycle.
  • pingdotgg/t3code#23175: The change covers blocking-wait behavior and delegation guidance, but does not establish concurrent dispatch for batched calls.
Merge Risk: πŸ”΅ Low Β· up to 8aadc

A parent may receive a notice for a request that has already ended. This narrow timing issue should be fixed or explicitly accepted before merge.

Security architecture risk: πŸ”΅ Low Β· up to bfd15

This changes when parent conversations resume and when approval reminders are withdrawn. The inspected paths preserve the existing human approval workflow; no new approval bypass was found. Restart recovery and some concurrent lifecycle cases remain incompletely verified.

Retained concerns No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred β€” The added influence is a child request causing a notice and potentially an agent turn in its owning parent conversation. The inspected gates constrain this path to the matching app-owned parent/child task relationship; they do not grant the parent new approval authority.

Trust Boundaries and Controls

  • observed β€” Agent-facing pending-request handlers enforce pending user_input requests before reaching the generic runtime-response command. MCP invocation also uses authenticated, capability-gated thread access. These existing controls, rather than notice wording alone, prevent this agent tool from approving command, permission, or MCP elicitation requests.

Resilience and Maintainability Implications

  • observed β€” Cleanup retracts queued notices only; an already-running notice is not retracted. Approval guidance therefore explicitly handles requests that are no longer pending. The added tests inspect duplicate delivery, resolution, Stop/disposal, late dispatch rejection, and deferred parent-run catch-up.

Use a named Effect.fn span, or explain Effect.fnUntraced.

cancelQueuedDelegatedRequests is a new reusable helper. It uses Effect.fnUntraced with no stated reason. Other untraced helpers in this file carry a comment such as "the enclosing operation owns the span". This helper is also called from several dispatch paths.

Use a named span. This is a new function, so no existing span name changes.

If you need an untraced helper, add a comment that states why.

As per path instructions: "reusable functions should generally use named Effect.fn spans; use Effect.fnUntraced deliberately and explain why".

  • docs(server): explain delegated cancellation tracing boundary8aadcc5

Addressed the tracing nitpick in 8aadcc59a9: the enclosing dispatch span owns this helper work, and individual cancellations already have a named span. Added the explanation without changing tracing or runtime behavior. Targeted formatting and diff checks pass.

Validate the child request before dispatching the notice.

The reactor can read a pending request, then the child request can end before dispatchMessage runs. The dispatch guard checks only that the delegated task remains live, so it accepts the notice. An idle parent can create a starting notice run, while withdrawal cancels only queued runs.

Carry the request ID in the notification source and reject the notice when that request is no longer pending.

Included review availability: This review used your included allowance. Your plan provides up to 10 included reviews per hour; 8 remain after this review.

The pending-read/dispatch race is possible, as on the existing delegated-question path. Approval notices explicitly say no action is needed if the request has resolved; actual request responses still validate pending state. Resolution withdraws queued notices, but does not interrupt an already-started parent turn.

Keeping these advisory semantics for this fix. Adding a persisted request ID and another child read would narrow the window without making delivery atomic: child resolution and parent dispatch use separate thread locks. No approval authority changes. Independent source review and all 18 focused control-read tests passed at 8aadcc59a9.
