A Claude continuation can remain running indefinitely when it receives only a background subagent completion and Claude never opens a wake turn. This blocks later messages until the server restarts.

The continuation now completes after two minutes without root wake activity. Root messages and streaming keep an active wake turn open; child progress and child messages do not. Deadline settlement shares the SDK-message lock, checks the owning query and turn, and preserves pending Stop requests.

Maintainer takeover of the fix by @AdEx-Partners-DE in #19196. The contributor commits are preserved and current main is merged. The session-wide frame counter was replaced with per-turn wake evidence.

Verification on head bfe0d93122:

  • Existing test file: original 3 PR tests replaced with 3 deterministic regressions; 2 additional regressions cover child-only buffered output and root streaming. No new test files.
  • Before: reverting the fix reproduces the missing deadline completion. The original PR also fails pending-Stop, child-only-buffer and root-streaming regressions.
  • After: all 291 tests in ClaudeAdapterV2.test.ts pass, including all 5 wake-deadline regressions.
  • Targeted lint, formatting, server typecheck, and diff whitespace checks pass.
  • Maintainer integration pass: all 15 tests in ClaudeReplayFixtures.integration.test.ts and BackgroundWorkStop.integration.test.ts pass.

One independent review round (Claude Opus 5.5 and GPT-6 Sol) found no blockers. Both noted that existing query-exit cleanup remains asynchronous; the exact deadline/process-exit overlap was not reproduced.

This is server behavior shared by web, desktop and mobile, with no wire or client changes. Proof uses deterministic adapter events and the virtual clock; screenshots are not applicable. The rare real-Claude timing sequence was not reproduced live. macOS, Windows, Electron desktop, iOS and Android were not run on this headless Linux machine.

Model and harness: GPT-6 Astra (tests), GPT-6.1 Sol (implementation and maintainer integration), Claude Opus 5.5 and GPT-6 Sol (independent reviews), through T3 Code.

Approvability

Verdict: Approved at 266dbde

Macroscope's review found this PR approvable — This is a small, self-contained server fix that bounds a previously indefinite Claude continuation and preserves active wake turns when SDK frames arrive. The accompanying tests cover both timeout settlement and normal wake-turn processing, with no schema, deployment, security, billing, or static-analysis changes.

📝 Walkthrough

The Claude adapter now waits up to two minutes for a root wake-turn frame before completing certain subagent continuations. Tests cover completion when no wake frame arrives, unrelated subagent progress, and continued waiting when a wake frame arrives.

Changes: Claude wake continuation

Layer / File(s)Summary
Continuation wait and validation
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The adapter tracks root-turn frames during the wait. If the drained continuation has no native wake frame or opaque-task notification that can settle immediately, it waits up to two minutes. It completes only if the same turn remains active and the root-frame count has not changed. Tests cover timeout without a root wake frame, unrelated subagent progress, and completion after a wake result when a root init arrives before the deadline.

Change: Bug fix

Merge Risk: ⚪ Minimal · up to b9666

The wake continuation can settle after the deadline without unrelated subagent progress keeping it open. No merge-blocking issue was established.

Security architecture risk: 🔵 Low · up to 266db

The change adds no access or privileges and improves recovery from silent background continuations. However, the new deadline can report completion while a stop request is still pending, leaving completion and cleanup insufficiently coordinated.

Retained concerns

  • Medium · reliability · inferred: The silent-wake deadline can publish completed status after Stop has been requested but before interruption finishes. interruptTurn records the interruption and then awaits interrupt and close operations; the deadline checks only active-context identity and SDK activity. Its completed settlement bypasses the non-completed cleanup branch that stops run-owned workflows. With a quiet, unresponsive query, successful completion and release of queued work can therefore precede resolution of the stop operation. This is confined to the affected continuation, not a demonstrated authorization bypass.
Security review details

Security Blast Radius

  • inferred — The directly evidenced failure scope is the captured session's continuation, its cleanup, and queued work. The counter is session-local and the timer checks exact context identity; no new cross-tenant, credential, or environment authority path was demonstrated.

Trust Boundaries and Controls

  • observed — Normal interruption validates provider-turn identity, and query-exit cleanup claims the live query by identity before touching its turn. The deadline is owned by the session scope, but does not consult the interruption marker used by normal result handling.

Resilience and Maintainability Implications

  • inferred — Deadline-versus-live-frame settlement remains insufficiently proven: live messages acquire the SDK semaphore, while the timer does not, and finalization emits terminal state before clearing active ownership. Precise scheduling and downstream containment of competing settlements were not established; this is not a verified exploit.
  • observed — Any handled SDK message permanently suppresses the one-shot deadline, even without native wake progress. This leaves an activity-interference case unresolved, but the full comparison shows indefinite waiting was already possible for these continuations.

Hardening Proposals

  • proposed — Use a single guarded settlement transition shared by deadline, interruption, and result paths. Atomically claim completion and preserve interruption semantics when Stop is pending, rather than relying only on caller-side identity and activity checks.

Keep the deadline active when a frame is not a wake-turn start.

If another subagent emits a progress frame during this wait, sdkFramesHandled changes. The deadline then returns without settling the continuation or scheduling another check. If Claude never opens the root wake turn, the run remains open indefinitely. Track evidence that the wake turn opened, rather than treating every SDK frame as that evidence.

✅ Addressed in commit b96662d

Valid, fixed in b96662d6a. The deadline now counts only frames of the root session's own turn (its init, or a root user/assistant message), via a new isClaudeRootTurnFrame, so a subagent's progress frame during the wait no longer counts as the wake turn opening.

Added the test "settles a subagent wake run although a subagent reports progress meanwhile": a task_progress frame arrives during the wait, and the run still settles after two minutes. With the previous counter it fails ("Timed out waiting for continuation terminal"); with the fix it passes. The whole ClaudeAdapterV2.test.ts (243 tests) and tsc --noEmit for apps/server pass on the fork with the same change.
