fix(server): Claude background subagents end when their wake can't run on Claude#17109

Claude background task notifications can arrive after a thread switches providers. The old continuation could dispatch into the newly selected provider and leave later subagents stuck running.

Buffered continuations now check provider instance ownership under the command lock. Queued wakes are cancelled before they can switch the thread back; cancelled or failed deliveries release their offer. Claude applies dropped outcomes through ordered session ingestion, preserving resumed generations and updating persisted results even after the original run stops ingesting. Active runs still receive lifecycle notifications so their subscriptions close. An ordinary artifact publication failure is logged without stopping the shared provider session; request-state failures, normalization errors and defects remain fatal. The failed artifact may stay stale; this adds no retry machinery.

The instance guards and cancellation/failure cleanup apply to all adapter-buffered wakes using each adapter's drop callback. For Pi, cancelling a queued wake or failing it before start terminates its pending native connection; this behavior was source-reviewed, not exercised live.

Based on @jieyuexing’s implementation in #17109. Contributor commits are preserved, with current main merged and maintainer fixes on top. The drop policy discards the old root wake output from the T3 timeline; Claude’s native session retains it. Opaque Bash notification projection and native-session release behavior remain outside this change.

Verification:

  • 495/495 tests across nine focused server suites. Five original PR cases became four behavior regressions; eight further cases cover atomic admission, queued switch/cancel/start failure, resumed transcript, subscription closure live-output cleanup and stored transcript ordering. Three additional fatal-boundary controls cover untagged requests, tagged request nodes and true defects. Existing main tests are preserved.
  • Before: all four initial regressions fail on main; the contributor implementation still fails provider switching after eligibility and resumed-result persistence (314/316 pass). After: all acceptance cases pass. Four later fixes were individually reverted to confirm their regressions fail.
  • Earlier R3 replay suites (not rerun for the manager-only R4 change): 158/161 passed initially; three load timeouts passed on unchanged-source targeted retry.
  • R4 actual EventSink/SQLite regression: 68 pass/1 fail on R3 → 69/69 after the fix, including three fatal controls. Later events persist on the same live runtime after a contained failure.
  • Server/provider-core typechecks, targeted lint, formatting and diff checks pass; existing lint warnings remain.
  • Five independent adversarial rounds: the first found queued lifecycle and session cleanup gaps. The second found transcript reordering from live turn-item session ingestion. Both were fixed; round three Opus and Sol report no code blockers. R4 fixed the later-confirmed artifact-persistence failure. Final round five Opus and Sol both report no code blockers.
  • Before/after test proof.

Earlier R3 live verification (UI unchanged by R4): an isolated Linux development server started and the collaborative browser paired and loaded the chat composer. The behavior above is verified through deterministic SDK frames and real adapter/session/orchestration/SQLite services. An authenticated live Claude CLI provider-switch session was not exercised. The live-data snapshot was incompatible with this checkout’s migration history, so the browser smoke check used fresh isolated state. No screenshot is attached because no visual component changed.

CI at b39ea65: all eight server shards, lint, build/typecheck, shared tests and fingerprint passed. CodeRabbit reports success; all review threads are resolved, and two obsolete bot change requests were dismissed with source-backed explanations. Web CI failed twice at unchanged AttachmentFilePreview.browser.test.tsx:74 (expected undefined to be '<p>Captured HTML</p>'; 8047/8048 tests pass), including one failed-job rerun. Failed rerun. No web files changed in this PR. This PR is blocked by web CI and is not merge-ready; no further retries requested.

Not verified: macOS, Windows, Electron desktop, iOS or Android. No merge performed.

Models: GPT-6 Astra for test reduction; GPT-6.1 Sol for implementation; Claude Opus 5.5 and GPT-6 Sol for adversarial reviews. Harness: Codex and Claude Code through T3 Code delegation.

📝 Walkthrough

Provider events now carry session-thread identifiers, and session-owned events follow updated ingestion and publication paths. Claude continuation buffers use generation-scoped cleanup. Continuation dispatch checks thread state and provider selection, including queued provider notifications.

Changes: Provider continuation lifecycle

Layer / File(s)Summary
Session-owned event routing
packages/provider-core/src/server/ProviderAdapter.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/ProviderSessionManager.ts, apps/server/src/orchestration-v2/RunExecutionService.ts, apps/server/src/orchestration-v2/ThreadManagementService.ts, apps/server/src/orchestration-v2/testkit/ProviderReplayHarness.ts, apps/server/src/orchestration-v2/ProviderSessionManager.test.ts, apps/server/src/orchestration-v2/RunExecutionService.test.ts
Provider events can carry a session thread ID. Session managers use it to ingest and publish session-owned events. Run execution skips normal ingestion for those events while still updating child lifecycle state. Tests cover session-owned command output and late child artifacts.
Generation-scoped Claude wake cleanup
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts, apps/server/src/orchestration-v2/ClaudeAutomaticDelivery.integration.test.ts
Claude continuation offers claim a buffer generation. Cleanup and replay operate on the matching generation, and dropped-wake handling applies the latest unapplied task notification. Tests cover failed dispatches, stale cleanup, resumed subagents, and assistant text ordering.
Continuation dispatch validation
apps/server/src/orchestration-v2/ProviderContinuationService.ts, apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/orchestration-v2/ThreadManagementService.ts, apps/server/src/orchestration-v2/ProviderContinuationService.test.ts, apps/server/src/orchestration-v2/Orchestrator.control-reads.test.ts, apps/server/src/orchestration-v2/ClaudeAutomaticDelivery.integration.test.ts
The continuation service tracks buffered requests, clears them on terminal outcomes or dispatch failure, and drops requests for archived, deleted, or mismatched provider threads. The orchestrator rejects stale linked dispatches and cancels queued provider notifications when provider selection changes. Tests cover provider switches, cancellation, and queued-start failures.

Merge Risk: ⚪ Minimal · up to b39ea

Older wake cleanup preserves newer offers, and resumed subagent results persist after the original run stops. No identified issue remains to block merging after normal checks.

Security architecture risk: 🔵 Low · up to b39ea

The changes add safeguards against stale background work running through the wrong provider and preserve completion handling after a run ends. No introduced security vulnerability is established by the available evidence. Some ownership and failure-recovery behavior could not be fully verified, so residual risk remains.

Retained concerns No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The demonstrated scope is provider-session event persistence, thread subscribers, and buffered continuation execution within the server. The shared event contract and adapter-buffered cleanup broaden the affected internal surface beyond a single Claude callback; the available evidence does not establish maximum cross-thread or tenant exposure.

Trust Boundaries and Controls

  • observed — The session manager gives explicit sessionThreadId routing precedence. The Claude producer summary ties this field to session context or selected child artifacts, but complete producer provenance and enforcement of session membership could not be verified. No model- or tool-controlled foreign-thread routing path was established.
  • observed — Runtime-request classification includes request updates, nodes carrying runtime-request IDs, and approval or user-input turn items. Their publication failures remain fatal; the new continuation behavior applies to ordinary session-owned artifacts instead.

Resilience and Maintainability Implications

  • observed — Terminal run events remove the matching buffered offer and invoke its cleanup callback; failed delivery also removes tracking and invokes cleanup. Ordinary session-artifact publication failure is logged without stopping the shared session, accepting potentially stale artifacts rather than providing retry or guaranteed recovery.
  • inferred — Generation checks and the resumed-completion assertion provide counterevidence to stale cleanup erasing a newer Claude offer or losing its durable terminal result. They do not prove all interruption, replay, cancellation, or native-resource cleanup paths, and test execution was not independently verified.

Make the generation check atomic with wake-state cleanup.

dropBufferedWake checks the generation, removes the continuation marker, then yields before removing the buffer. A new bufferWakeMessage can run in that window, create a newer generation, and offer a new continuation. The old drop can then delete the newer buffer.

Store the generation with both the continuation marker and wake buffer, and validate it inside the atomic state update. Keying only requestedContinuations is not sufficient because wakeBuffers is updated separately.

Already addressed before this round: in 710f1ea482 the generation check and the buffer removal happen in one Ref.modify(wakeBuffers), and the offer's generation is assigned in the same Ref; there is no separate requestedContinuations cleanup anymore. The existing regression ("an older drop preserves the newer wake") covers it.

Rebased onto current main (5047ee7885) and squashed into one commit. The conflict was with the Claude workflow-progress handler from #12598. That handler is kept, along with the newer subagent stop (#17826), effort (#17496) and rate-limit (#17778) behavior. Dropped notifications now go through the existing claudeTaskOutcome mapping and carry their workflow frame.

On main the original bug still reproduces: a buffered wake dispatches after the thread switches provider instance, and Claude's offer has no drop callback.

CodeRabbit's race finding was right, and the race was in this PR's own drop path. The drop checked the generation, removed the continuation marker, and only then removed the buffer, so a new wake could be offered in between and the old drop would delete it. The offer generation now lives on the wake-buffer entry, and a drop checks it and removes the buffer in one Ref.modify. This also removes the separate requestedContinuations set. I reproduced the interleaving locally with a controlled Effect scheduler. That fixture depended on Effect's internal op counts, so it is not committed. The committed test checks the rule the fix sets: a stale drop leaves a newer wake alone. It also passes on the code before this fix, because a drop that runs after a newer offer was already caught by the top-level generation check.

ProviderContinuationService.test.ts, ClaudeAdapterV2.test.ts and RestartContinuation.test.ts pass (275 tests). Targeted fmt and the apps/server typecheck are clean. Targeted lint reports only the existing unused layer warning.

Not covered: no live Claude session with a provider switch.

Deliver dropped resumed-subagent outcomes through an active ingestion path.

When a completed subagent resumes while the root is idle, bufferWakeMessage reopens only the session registry and buffers task_started. If the wake is dropped, dropBufferedWake skips that frame and sends the terminal notification through the last settled context. updateClaudeSubagentNode then keeps the earlier runId.

RunExecutionService routes parent-thread subagent.updated events by runId. The earlier run can stop ingesting after its child terminalizes, so the dropped terminal event can be ignored. The projection can remain at the prior terminal state and miss the resumed subagent's new outcome.

Route the dropped outcome through a live ingestion path, or persist it independently of the earlier run. Do not rely on the settled run to ingest this update.

Agreed that the source path supports this, though I haven't reproduced the missing projection end to end. Idle task_started only reopens the session registry and buffers the frame; a dropped outcome goes out through the last settled context and keeps the earlier runId, and RunExecutionService filters parent-thread subagent events by run and stops subscribing once that run settles with no tracked background work.

Re-pointing the runId alone wouldn't guarantee a live subscriber; a real fix needs an explicit ingestion or persistence owner for idle resumed outcomes, which reaches beyond the adapter. Since drop versus defer is still waiting on a maintainer, I'd rather not widen this PR before that's decided. If drop is confirmed I can add it here or as a follow-up, whichever the maintainer prefers.

Fixed in the maintainer takeover, now at 7ac31eebc8. Dropped outcomes carry session ownership and are persisted by ProviderSessionManager independently of the settled run. They are then published for any remaining run subscription to track lifecycle without ingesting them twice. The integration case persists a resumed subagent outcome when its buffered wake is dropped drives the real adapter, session manager, orchestrator and SQLite projection; it fails on main and on the contributor implementation, and passes on this head. A separate regression verifies that live turn-item routing preserves the introduction/card order. Both Opus and Sol completed a clean full-PR third review. Current before/after proof.

Guard dropped outcomes against a newer task_started.

dropBufferedWake removes the old buffer, then applies its buffered task_notification messages. A newer task_started can arrive between these operations. The new start is buffered, but the registry remains running, so the older notification can replace it with a terminal state. Serialize the lifecycle check with SDK frame handling and skip the dropped notification when a newer task_started exists for the task.

Add a regression test that pauses after Ref.modify, delivers the new task_started, and then resumes the drop.

✅ Addressed in commit d06173b

Note

Drafted by Codex (gpt-6.1-sol) and Claude Opus 5.5 in T3 Code; reviewed and posted by @jieyuexing

Confirmed and fixed in d06173bf6a. The drop now holds the same SDK frame semaphore that live frames and replay use for the whole detach-and-apply step, and applies only the latest lifecycle frame per task from the detached buffer. A new regression covers both orderings: the newer task_started already buffered, and arriving after the buffer is detached but before the terminal state is published. Both failed on 710f1ea482 and pass now.

Note

This comment is posted by Julius' dot

The tests establish the cross-provider wake bug, but the discussion in #17099 leaves dropping versus deferring the buffered output to maintainers. This PR chooses drop and removes that output from the T3 timeline. Can a maintainer confirm that direction under the scope policy?

Synced from GitHub

Note

Drafted by Claude Opus 5.5 in Claude Code (running inside T3 Code); reviewed and posted by @jieyuexing

For the maintainer deciding this: drop follows the analysis in #17099 (https://github.com/pingdotgg/t3code/issues/17099#issuecomment-6054395158), which left drop vs. defer to maintainers and called drop the lower-risk default. Drop reuses the archived-thread contract, every subagent reaches a terminal state, and the wake output stays in Claude's native session for when the thread resumes on Claude. Defer would keep the output in the T3 timeline, but it needs durable pending state and never ends if the user doesn't switch back.

If you prefer defer, I'll rework the PR that way. The two new CodeRabbit findings only apply to the drop path, so I'll hold them until the direction is settled.

Do not release the provider session when session-owned persistence fails.

ClaudeAdapterV2 attaches sessionThreadId to session-owned events. ProviderSessionManager persists these events through ingestNormalized. A persistence failure becomes ProviderAdapterEventStreamError, and the event-pump exit path releases the provider session. This removes the live session, fails its subscribers, and closes its scope. One failed subagent or node write can therefore stop the session's other background work.

Catch failures for events with sessionThreadId and keep the pump running. Keep fallback runtime-request writes fatal.

✅ Confirmed as addressed by @juliusmarminge

Fixed in b39ea65bb0. The catch is narrower than the suggestion: only typed ProviderEventPublishError at ingestNormalized is contained, only for explicitly session-owned non-request artifacts. Semantic request rows/nodes/approval/input cards remain fatal even when tagged and carrying a run ID; normalization failures, defects and interruption also retain their existing paths. The real SQLite/EventSink regression is 68 pass/1 fail before -> 69/69 after, including three fatal-boundary controls. The nine-suite focused set passes 495/495. A failed artifact can remain stale; no retry or durable recovery is claimed. Current proof: https://gh-file-drop-api-prod-mi5fy3sowv63ufte.pinglabs.workers.dev/f/c014fdb25108c20e/17109-proof-r4.md

@juliusmarminge Thanks for the fix and clarification. The provided code contains only typed ProviderEventPublishError failures for explicitly session-owned, non-request artifacts. This addresses the session-release concern without suppressing request persistence failures, normalization failures, defects, or interruption.

The narrower catch is appropriate. It contains the failure but does not guarantee recovery of the failed artifact. I have not independently run the reported tests.
