Some server services were split in two: the Context.Service tag lived in Services/Foo.ts and its layer in Layers/Foo.ts. Callers had to import both halves under two names, like ProviderRegistryLayer next to ProviderRegistry. docs/internals/effect-services.md asks for one module per service, with the interface written inline in the tag and the module imported once as a namespace (ProviderRegistry.ProviderRegistry, ProviderRegistry.layer).

This PR merges the split services and moves everything else in Services/ and Layers/ up into the domain folder. Both folders are gone.

Each merged module follows the doc's order: imports, schemas/types, the tag with its interface inline, make, then layer.

The separate …Shape interfaces are removed. The few places that used one now use Foo.Foo["Service"].
makeProviderAuthService and makeProviderInstanceRegistry stay exported because tests import them.

The namespace aliases that worked around the split are collapsed into the single module import:

Every file in provider/Layers/ → provider/. That covers the per-provider *Provider.ts files, catalogs, loggers, ProviderInstanceRegistryHydration, ProviderUsageLimitsIngestion, the usage-limit and reset-credit helpers, and their tests. provider/Layers/testing/ → provider/testing/
persistence/Layers/Sqlite.ts (+ test) and the OrchestrationEventStore tests → persistence/
observability/Layers/Observability.ts → observability/

None of these names already existed at the destination.

ProviderInstanceRegistry decision

Merged: the registry tag and its live implementation. Its only layer, which also provides the mutator, is plain ProviderInstanceRegistry.layer.
Kept as their own modules at provider/ root:
- ProviderInstanceRegistryMutator is a separate service with its own tag. It stays separate on purpose, so read-only consumers can't reconcile.
- ProviderInstanceRegistryHydration (.layer, deriveProviderInstanceConfigMap) can't go into the registry module. It imports builtInDrivers, and the drivers import ProviderInstanceRegistry through ClaudeDriver → ClaudeTextGeneration → TextGeneration, so merging would create an import cycle.

Behavior

Nothing changes at runtime. The only edits beyond paths and types:

Service key strings moved with the files, e.g. t3/provider/Services/ProviderRegistry → t3/provider/ProviderRegistry. The same applies to CursorSdkCatalog, ProviderEventLoggers and resetCreditCoordinator. The Effect language service's deterministicKeys rule fails typecheck otherwise. These keys only identify services inside the server process; they are never stored or sent to clients.
Test fixture paths: two tests built fixture paths from their own folder (import.meta.dirname), so those moved up one level.
Shard weights: shardWeights.json keys were renamed and re-sorted.
Docs: two doc links, a script comment and a .cursor rule path were updated.

The first commit is pure git mv, so GitHub shows those files as renames. The second does the merges and import rewrites. Import paths were rewritten with a TypeScript-AST codemod, not regex.
