fix(server): restore usage-limit recovery CI checks

Remove PR-only capture scripts and their unused recipe; published evidence remains linked from the PR.

The query-plan test counts indexed error lookups, not persisted payload rows. Preserving the limited source adds a latest-failure lookup to each read query (three shell lookups, two recovery lookups). Scope it by thread so all lookups retain the failed-error composite index and update the exact counts without relaxing index assertions. No failure write path changes.
