Merge PR #20928 delegated approval notices into fix/issue-22925

Keeps the PR's Question->Request reactor rename, its stop/dispose
cancellation guards and tests, main's scoped projection reads, and the
live-only approval predicate and catch-up path. The command-approval
repro folds into the PR's approval test as a parametrized case.
