fix(server): refresh PR metadata with Git fetch disabled

Credit Kevin Beier (@tonoizer) for the original metadata-polling proposal in [#4108](https://github.com/pingdotgg/t3code/pull/4108). This narrowed port preserves current lookup and background policy behavior.
