fix(web): pending questions become a question-first menu

The distinct header demotes to a quiet 12px label (it already titles the timeline row) and the question leads at 14px in both titled and untitled cases. Options become single-line menu rows: label and description in baseline-aligned columns, fills hanging 12px past the text column so labels share the question's edge, indicators and the caret on one right axis. Selected rows use bg-foreground/8 (bg-muted/55 computed to ~1.65% white in dark). The multi-select hint folds into the eyebrow or the actions row. Mobile mirrors the eyebrow/question hierarchy with stacked descriptions.
