fix(server): ingestion failure replays a terminal root provider turn

The root snapshot is cached before it is published, so the failure path
cannot tell whether a terminal snapshot was stored. Replay it unchanged
instead of overwriting it or skipping it; active turns still end as
interrupted or failed.
