fix(codex): keep unconfirmed 429 descendants out of idle

When the 429 deadline or the unconfirmed-stop bound interrupts a
descendant and Codex never acknowledges that interrupt, mark the
descendant abandoned before finalizing it so its thread is published as
errored instead of idle while the native turn may still be running.

```
