fix(web): center pending question content and drop generic header

The pending-question body inherited the banner's icon indent (32px left) against only 8px on the right, so options and question text sat visibly off-center. Give the panel symmetric 16px surface insets in both overflow and non-overflow scroll states. Also hide the generic Question/Question N header on web and mobile: expanded panels show only the utility controls, collapsed panels show the question itself, and distinct provider headers render as before.
