fix(server): show Claude MCP elicitations on the approval card

The Claude adapter never registered the Agent SDK's onElicitation callback,
so the SDK declined every MCP elicitation before the user saw it.

Approval-only forms now open the existing mcp-elicitation card with explicit
Cancel, Decline and Approve options and wait for the answer. The form is
filled with the Codex helper, so what counts as approval-only matches Codex,
but schema defaults are never sent since the card doesn't show them. URL
mode and forms asking for values the card can't collect are declined
without a card.
