fix(server): Codex turns survive a renamed permissions profile, and forks keep it (#20266)

A turn/start that selected a remembered profile Codex no longer knows now
forgets the profile and retries once with the legacy sandboxPolicy. Forked
threads remember the profile from the thread/fork response.
