Support remote hosts for WS transport and Vite dev server

- resolve WebSocket URL with explicit/bridge/env/page fallback and token propagation
- add wsTransport tests for URL precedence and token handling
- add remote-access guide and README section for cross-device usage
- add `T3CODE_PUBLIC_HOST` wiring in dev runner plus Vite host/HMR overrides
