Adopt JSON-first document schema in SQLite persistence plan

- Replace normalized table proposal with a `documents` envelope + `data_json` model
- Update indexing, migration phases, test scope, and done criteria for document-based persistence
- Add schema-versioning and JSON shape drift mitigations
