fix(codex): keep an unconfirmed 429 turn uncontained until turn/completed

A turn/interrupt reply only accepts the request. Stop and the late
descendant pass treated Codex's reply as confirmation, so a 429 turn still
waiting unconfirmed was published reusable (root) or idle (subagent) while
Codex could still be retrying it natively.

Treat "unconfirmed" as uncontained regardless of interrupt replies; only
turn/completed moves the turn to "ended". Drop the now-unused
rateLimitStopAcknowledged flag.
