test(server): add usage-limit recovery web evidence scripts

Seed synthetic usage-limit threads (W0, W1, W2, W4, W5, W7, W8, plus
held-queue and running-wake stages) into a disposable T3 home, then
capture web stills and recordings against a dev server. Every usage
limit is a synthetic fixture; continuations run on the Codex mock peer.
