Fix ProviderCommandReactor: add error recovery and deduplicate CWD resolution

- Wrap processDomainEvent in catchCause to prevent the reactor fiber from
  terminating on handler errors, matching the pattern used by
  ProviderRuntimeIngestion and CheckpointReactor.
- Replace duplicated resolveEffectiveThreadCwd with the shared
  resolveThreadWorkspaceCwd from checkpointing/Utils.ts.
