fix(server): provider installs wait for turns on every T3 server on the host

A user-wide reader/writer gate in the user cache coordinates independent
T3 processes. Installs take the write lease; turns, compaction, session
operations and text generation take read leases that last until their
background work drains. Crashed holders are reclaimed by PID identity.
