Adds t3code/no-test-timeout-or-unreasoned-skip: per-test timeout/retry
arguments, hook timeouts, and vi.setConfig timeouts are errors, and
.skip/.todo/.only/xit/fit need a `// quarantine: #issue` comment.
Opt-in *.live.test.* provider suites keep their timeouts.

Removes every existing override, pins retry: 0, allowOnly outside CI, and
the full fake-timer set in the root test config, and spells out the
no-timing rules in AGENTS.md.
