fix(codex): publish an uncontained 429 root thread as errored

A root turn whose 429 interrupt Codex never confirmed ends with a broken
thread disposition, but that only reaches the projection. A provider
session that stays live drops a loaded thread only when it sees the
thread errored, so the next turn could start directly on the native
thread that may still be retrying. Publish the root thread as errored
before its terminal, as the subagent path already does, so the next turn
resumes it instead.
