fix(server): agent messages no longer cancel Claude tools

Queue agent-authored updates when the live provider session declares that
active steering interrupts tools. This covers automatic delivery and explicit
steer requests while preserving human steering and intentional restarts.

Explain the fallback in the MCP tool description and verify queued delivery,
human steering, and safe-provider steering through the orchestrator.
