fix(web): pending questions get a title slot and calmer selection

Distinct headers now lead as a normal-case title with the question as secondary info below it, all on one 16px column with the option rows. Dividers are gone; selection is back to the original soft row highlight with a fuller check on the right (number swaps in the same fixed slot). Mobile mirrors the same hierarchy.
