fix(server): older clients no longer show background provider installs as manual updates

Current clients opt into provider update origins on the config subscription. The
server strips background update state from snapshots and status events sent to
clients that do not. Docs describe recognized-installation limits, the manual
native fallback, and host-wide lease ordering.
