fix(server): a subagent of an earlier completed run keeps asking after a later run settles

A running subagent stays with the run that launched it, and that run keeps
ingesting its events. The settled fallback only remembered the latest
completed run, so once a wake continuation or a later turn settled, the
earlier run's still-running subagent had every permission request denied.
Keep the settled context per completed run and raise the approval in the
subagent's own run.
