refactor(web): the composer's layout and provider hooks live in their own files

Moves the resting-transition animation hook, the command menu layer, the
resting controls layout hook, the workspace snapshot refresh hook, the
provider/model resolution hook and the image grouping hook out of
ChatComposer.tsx unchanged.
