test(server): reproduces issue 16636, a wedged provider holds every effect worker

Four provider-turn.start effects whose executor never resolves occupy all four
default effect workers. Fifteen virtual minutes later, far past the 30s lease,
no deadline has freed a slot and a Claude provider-turn.start behind them never
runs. Red only; no fix.
