Add desktop DMG packaging and persisted app settings

- package desktop builds with branded icons, custom app identity, and `t3://app` production loading
- add app settings get/update RPC flow with persistence and tests for Codex binary/home paths
- wire web/desktop settings UX and menu action hooks to open Settings
