fix(opencode): actionable error when a session outgrows the model window

OpenCode reports an oversized session as a bare InvalidRequest, so the
run failed with the provider's generic 'invalid parameters' text and
every retry failed identically. When the window is known and this
turn's observed input already meets it, fail with what actually
happened plus the recovery (fork from an earlier completed run or
start fresh). Anything without size evidence keeps the original error.
