agents: reload agent manifests without restarting the server

agents.list / agents.reload (and `signaltty agents list|reload`) show and
re-read the agents dir. Ctx holds the manifests as an Arc snapshot that a
reload swaps whole, so a request in flight sees one consistent set and live
panes keep their process and kind. Malformed files are listed, not fatal.
