SIGNALTTY_INTEGRATION_HOME= (and Hooks::from_env with an empty path)
used the empty string as the provider write root, so install wrote
relative files like .codex/hooks.json under the process cwd. Empty now
means unset — fall back to HOME — matching #67/#74/#75 for other path
envs. Config likewise ignores an empty SIGNALTTY_INTEGRATION_HOME.
