gui: refine visual hierarchy, card elevation, and theme adaptation

- Replace invalid GTK CSS @media query with libadwaita semantic colors (--card-shade-color and color mix)
- Preserve physical card drop shadows across all attention states (unread, input, permission, warning, error)
- Fix double border between pane header and decision bar, and tint decision bar according to pane attention level
- Harmonize typography scale across sidebar, panes, status pills, and dialogs
- Add subtle halos on active attention dots and speaking chip in sidebar
- Adopt calm, GNOME-native neutral selection for sidebar rows
- Add subtle paragraph backgrounds to diff added/removed lines in changes dialog
- Align command palette boxed list to top
