plugin: attribute hook stats by name after reload

dispatch recorded runs/errors via plugin and hook indices captured at
spawn time. plugin.reload replaces that vec (and resets stats), so an
in-flight hook could credit a different package after reorder or
removal. Look up the live plugin by name and matching hook identity
instead.
