* gui: polish workspace panel and generate first-prompt AI titles

* server: disable Codex MCP servers after the exec subcommand

Codex ignores root-level --config overrides once the exec subcommand
carries its own --config flags, so the mcp_servers override placed
before exec left the user's configured MCP servers running during
title generation. Pass it after exec with the other overrides and add
a regression test that rejects an override placed before exec.
