fix(claude): recognize percentage-only usage confidence (#526)

Preserve the official percentOnly confidence marker and show limited-detail text and a distinct panel badge without changing incomplete-quota guards.

Add regressions, translations, synthetic smoke scenarios and maintained behavior documentation. Track the independent Claude Platform icon gap found on the base commit with CLI 0.76.0 in TODO.md.
