fix(popup): clarify overview quota hierarchy

Separate quota names from reset metadata and anchor provider identity to the header. Keep hide actions immediately visible on hover and keyboard focus.

Documentation: update the usage guide, Unreleased changelog and synthetic Overview screenshot. TODO is unchanged because no listed parity item or CLI blocker is completed.
