Fix notification baselines, severed QML bindings, and formatting bugs

- NotificationPlanner: key quota/pace/reset rows by lane+identity plus a
  duplicate occurrence ordinal instead of the absolute row index, so
  inserting or reordering usage windows no longer re-announces quota and
  pace state or silently disarms pending reset notices.
- SpendView: restore the ComboBox currentIndex bindings after an
  interactive pick so range/metric keep tracking settings changes made
  outside the popup.
- configProviders/configDebug: reinstall the visibility binding that
  Kirigami's inline-message close button severs, so dismissing one banner
  does not hide every later message; dismissals clear their message text.
- configProviders: restore the descriptor enum combo binding after a pick
  and show display names in toggle/API-key errors.
- configDebug: report failed diagnostics that produce no stderr.
- SessionsView: refresh the session-age clock when the tab becomes
  visible again instead of rendering stale ages until the next tick.
- CostPresentation: promote token counts across unit boundaries after
  rounding ('1M' instead of '1000K') and degrade non-numeric Quota
  balances to '-' like every other currency.
- main.qml: drop the unused providerFallbackActive property.
