fix: correct provider ordering, cost history and CLI handling (#146)

Preserve disabled provider slots and live-provider capacity when reordering; reject ambiguous duplicate rosters. Inspect bounded cost history before filling gaps, preserving valid amounts and malformed-day semantics. Report empty provider fallback replies and distinguish CLI prereleases from release builds.

Validated with make check, full Plasma CI, package build, popup smoke tests, and final Codex review.
```
